The post holder will actively participate in formal job planning process in accordance with Trust Policy. This will be undertaken on appointment and reviewed on an annual basis, unless there are changes during the year. All changes made will be through bilateral consultation and negotiation between the appointee and the Clinical Director. The process will discuss and agree all aspects of appointee’s professional practice including clinical work, teaching, research and education. It will provide a clear schedule of commitments, both internal and external. The Consultant will play a pivotal role in ensuring proactive, recovery-focused, individual treatment plans are in place and being followed for each patient. The post holder for Northamptonshire Health foundation Trust will primarily have duties at Danetre and Turn Furlong with reciprocal cover for other sites. The role primarily involves looking after elderly patients in two community hospital sites admitted for rehabilitation purpose. This involves managing acute and chronic conditions in the context of multi-morbidity, frailty and polypharmacy working along with Multi-Disciplinary teams. ### Main duties of the job - Full Range of Medical Duties at Danetre and Turn Furlong and reciprocal cover for other sites. - Provide clinical leadership to the team and take clinical responsibility for the assessment and treatment of patients as part of a multidisciplinary clinical team. - Undertake clinical sessions on the ward and attend to all relevant matters there as a senior clinician and leader within that service. - Ward rounds - Supervision of Resident Doctors ### Detailed job description and main responsibilities The post is 0.8 WTE. This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service. ## Person Specification ### Skills **Essential** - Excellent written and verbal communication skills and evidence of the ability to communicate with patients, colleagues and staff at all levels. - Ability to motivate and develop junior Medical Staff and other MDT members - An understanding of the political and financial framework within which the Services operate - Flexible to cross cover other sites - Arrange transport to cover countywide **Desirable** - Wide range of specialist and sub-specialist experience relevant to post within NHS or comparable service - Able to plan and deliver undergraduate teaching and training relevant to this post - Experienced in clinical research and / or service evaluation ### Knowledge **Essential** - MRCP or equivalent - Full GMC Registration with CCT in GIM, geriatrics or palliative medicine or be within 6 months of award of CCT by date of interview or on specialist register via CESR - Experience in community geriatrics and general palliative care. **Desirable** - Additional Postgraduate Qualifications, eg.
