### JOB SUMMARY

To recruit a Consultant colleague, who will be innovative, driven and flexible, working within a successful multi-professional team. The successful candidates will join the existing team to provide a full range of inpatient and outpatient Obstetric services.

This post is based in the Ulster Hospital for labour ward sessions, inpatient care and on-call. Outpatient sessions will be in the Ulster Hospital or Lagan Valley Hospital. In the future, other sessions may be in other South Eastern Trust Hospitals and may change depending upon final job plan. The successful applicants will join a 1 in 12 first on call rota, providing emergency obstetric services.

This a full time post, however applications from practitioners who wish to contract for a reduced number of planned activities, or on a job share basis are welcome.

Profile of the South Eastern Health and Social Care Trust

The South Eastern Health & Social Care Trust (SET) is an Investors In People (IIP) accredited organisation, employing approximately 12,500 Staff.

It is an integrated Trust, incorporating acute hospital services, community health and social services. The Trust serves a population of 440,000 people and covers a wide geographical area from Bangor in the north, Portaferry in the East, to Newcastle in the South and Moira in the West. The 5 main hospital bases are the Ulster, Lagan Valley, Ards, Downe and Bangor Community Hospitals.

The Trust is the first in Northern Ireland to move to fully electronic patient records in November 2023 with Encompass.

Location and Surrounding Amenities

The Ulster Hospital is situated within Ards and North Down Borough Council, with the stunning backdrop of the North Down coast and the Ards peninsula. It is ideally located for urban, rural or semi-rural commuting. Belfast City centre is approximately 20 minutes away by public transport. George Best airport is 15 minutes away. The area has a large number of highly rated schools, attractive property buying opportunities and the area boasts many excellent sporting venues, such as Royal Belfast Golf Club, Ulster Rugby stadium and Royal North of Ireland Yacht Club.

Leisure opportunities are extensive with many excellent restaurants and other amenities available in close proximity to the hospital.

The Ulster Hospital

The Ulster Hospital is the major acute hospital for the Trust and delivers the full range of acute services for the population. This includes a type 1 Emergency Department which is open 24/7 (22/23 ~115k new & unplanned attendances), a comprehensive range of diagnostic services, the full range of outpatient, inpatient and daycase medical and surgical services, cancer care, coronary care, obstetrics, paediatric services and a psychiatric inpatient unit. The regional Plastic and Maxillo-Facial services are mainly delivered from the Ulster site.

The Ulster Hospital has undergone significant redevelopment with the building of a permanent renal unit, new maternity hospital, a new inpatient ward block and most recently an acute services ward block recently opened to add to, a new theatre block and Critical Care Complex that opened in November 2010.

The new inpatient ward block (IWB) was opened in February 2017 with 288 single en-suite rooms, 4 day surgery theatres and 3 endoscopy rooms.

The Acute Services Ward Block (ASB) is now operational. It includes 213 single en-suite rooms, a new X-ray department including CT, MRI and Nuclear Medicine imaging. The Emergency Department, which completes the final stage of the £280m Phase B redevelopment project, opened on the 6 September 2023.

The Ulster Hospital is a major teaching hospital associated with Queens University of Belfast and with the Ulster University. The library is linked to Queens University Medical Library and comprehensive on-line search facilities and other resources are provided through library connections. Facilities for Med-line and Cochrane Database are also available. Research opportunities are also being developed, particularly in the field of bronchiectasis, lung cancer and health related technology.

Lagan Valley Hospital

Lagan Valley Hospital is located in the city of Lisburn, a 30-minute commute from the Ulster Hospital. It is situated close the M1 motorway, with easy access from Belfast. The Emergency Department team operate an Urgent Care Centre, which is a phone first system (appointment only), which is open 8am-6pm Monday-Friday (closed at weekends).

The hospital has 84 inpatient medical beds, a psychiatric inpatient unit and a Dementia Assessment and Treatment Unit, as well as outpatient, diagnostic, day-case services for the local population. It also provides maternity services, including Consultant lead antenatal clinics and a range of outpatient gynaecology services including general gynaecology clinics, a recurrent pregnancy loss clinic, colposcopy clinics and outpatient hysteroscopy clinics. The Midwifery led birthing Centres are currently suspended and under review at Lagan Valley Hospital and Downe Hospital.

Downe Hospital

The Downe Hospital is located in the culturally historic town of Downpatrick, a 40-minute commute from the Ulster Hospital. The Emergency Department team operate an Urgent Care Centre which is open 8am-6pm Monday-Friday. There is also a Minor Injury Service on Saturday and Sunday 9am-5pm.

The hospital has 40 inpatient medical beds, a psychiatric inpatient unit and a Dementia Assessment and Treatment Unit. A Frail Elderly Rapid Assessment Clinic is staffed 9am to 6pm Monday to Friday. The Downe provides the Trust wide Bowel Screening Service and Sexual Health Service, as well as a range of outpatient, diagnostics and day-case services, such as cataract surgery. Two Primary Care practices are located on the first floor, along with community and dental services. There is a consultant led antenatal clinic and gynaecology outpatient and colposcopy clinic in this hospital.

Ards Hospital

Ards Community Hospital provides a comprehensive range of health, primary and social care services, including an X-Ray Department, GP Ward, Medical, Surgical, Gynaecology and Psychiatry Outpatients Department and GP Out of Hours services. The Trust’s breast screening service is delivered from a new building on the site.

Bangor Community Hospital

Bangor Community Hospital provides a range of health, social and primary care services, including an Outpatients Department, a Therapy & Rehabilitation Centre, and an X-Ray Department. Gynaecology outpatients and urogynaecology clinics are held on this site. The continence service for the area is based in Bangor Hospital and urodynamic studies are performed on an outpatients basis.

### Woman and Acute Child Sub Directorate

The Maternity and Gynaecology service is a Sub Directorate of the Woman and Acute Child Health Directorate with children’s services, hospital and community midwifery.

Intrapartum and Acute Obstetric care: Ulster Hospital site

Within the Ulster Hospital Maternity Unit there are 32 obstetric beds, a consultant led Labour Ward with 7 birthing rooms, including a birthing room with a dedicated bereavement suite, a post-operative recovery and level 2 care area, which has recently been expanded by three beds and 2 operating theatres. There is upwards on 4000 deliveries per annum. Approximately 800 of these deliveries occur in the 7 birthing rooms in the Home from Home Midwifery Led Unit. Midwifery led care is the normal for low risk women and the consultant led antenatal clinics are for high risk patients.

There is a 24 hour epidural service and dedicated consultant anaesthetic presence 45 hours per week.

We currently have a second Obstetric Consultant of the day to support the first on call / labour ward consultant. The Obstetric Consultant of the Day is present from 08:30 to 16:30 each normal working day. There is also a Gynaecology Consultant of the day is present 08:30 to 16:30.The model for acute/emergency consultant care is currently under review. The successful appointee will work opposite a consultant gynaecologist at all times, including on call. The Consultant gynaecologist will be available to assist when required for complex obstetric surgical procedures, such as massive obstetric haemorrhage.

Neonatology: Ulster Hospital site

There is a neonatal unit which has 13 cots, 2 level 1, 2 level 2 and 9 level 3 cots. There is a 24 hour, resident paediatric service. Elective neonatal care is provided from 27 weeks gestation and the neonatal unit is part of the Province wide Neonatal network.

Gynaecology

The Gynaecology Ward is now located in the Acute Services Block (Ward 4E) and has 18 beds. Intensive care is situated within the Critical care Complex..

### Abortion Service

The Trust offers medical and surgical abortion services and hosts the Regional Surgical Abortion Service.

The Tulip service is based in Lagan Valley Hospital, consultations for all patients accessing service occur there, as well as management of first trimester medical abortion and outpatient surgical management. The surgical abortion list is hosted in the Day Procedure Unit in the Ulster Hospital. One of our successful appointees will join this service to expand and support the current team.

A Gynae Hub Service and a rapid access service to support emergency department patient flow are currently being set up.

### Gynae Assessment and Early Pregnancy Service

Located within 4E, combined Consultant and Nurse Lead services are provided.

### Specialist Obstetric Outpatients

Ulster Hospital site.

Outpatient clinics occur in the Outpatient Department in the Maternity Unit. Midwifery led and consultant high risk antenatal clinics are provided.

### Obstetric Haematology Clinic

Joint care with a consultant haematologist for high risk women with haematological conditions Supported by a Speciality doctor and 2 resident higher trainees in Obstetrics

Obstetric Perinatal Mental Health Clinic,

Joint care with the PNMH team. Lead by a Consultant with a special interest in Perinatal Mental Health. The Specialist Perinatal Mental Health Midwife supports this clinic. Monthly MDM. Supported by a Specialty doctor and 2 resident higher trainees in Obstetrics

### Obstetric Diabetic and Endocrine Clinic

Joint clinic with the specialist endocrinology team. This clinic manages all type 1 and type 2 diabetics, as well as gestational diabetics. Weekly MDM. Supported by a speciality doctor and up to 2 resident higher trainees in Obstetrics

### Multiple Birth Clinic

Currently run by the Fetal Medicine Subspecialist. Supported by a Speciality doctor and up to 2 resident higher trainees in Obstetrics. The Consultant attends the weekly, regional MDM.

### Fetal Medicine Service

Currently there is one fetal medicine clinic per week, run by a subspecialist Consultant in fetal medicine. Another consultant has a small amount of job planned time to support with attendance at the regional, weekly MDT and to help with case planning when the subspecialist is on leave. We work closely with the regional Fetal Medicine Service. We will expand this service with a new appointment.

### Obstetric Placental Clinic

Lead by 2 consultants with a special interest and supported by a speciality doctor and 2 resident, higher speciality trainees in Obstetrics.

### Obstetric Clinic incorporating care of women with epilepsy

Incorporated within a general high risk obstetric clinic and lead by consultant with an interest. Supported by a speciality doctor and 2 resident, higher trainees.

High Risk Preterm birth Clinic

Women deemed high risk for preterm birth are seen by a consultant with a special interest. This is currently a single handed clinic and there is scope for expansion.

### Emergency Obstetric Unit

A busy 3 bedded/ 2 recliner chair unit. All emergency attendances are managed in this area. In addition this service provides elective antenatal care procedures such as Glucose tolerance tests, fetal assessment, and parenteral iron infusions. Currently staffed by a team of midwives, maternity support workers and middle grade doctors. We are currently reviewing this service, and dependent on some capital refurbishment, we intend to run a separate Emergency Obstetric Service and an Elective Ambulatory Obstetric Care service.

### Postnatal Review Clinic

One of the successful applicants will set up and lead a postnatal review clinic in collaboration with our Consultant Midwife and Head of Midwifery. This will be a new and much needed ensuring that women who have had a complicated birth are reviewed in a timely manner with the multidisciplinary team.

### Specialist Clinical Psychology Service

From April 2025 we have a dedicated Consultant Clinical Psychology service for maternity and neonatology. This will be led by a Consultant with a perinatal special interest and will be supported by a clinical psychologist.

### Other Antenatal services

Specialist midwife Doppler fetal assessment

Midwifery growth scanning clinics

ECV clinic. This is run by a speciality doctor.

Modern ultrasound equipment is available for all clinics.

### Specialist midwives for high risk antenatal service

We have a fetal surveillance specialist midwife, a perinatal mental health specialist midwife and an antenatal screening specialist midwife.

### Lagan Valley Hospital

### General high risk Consultant clinics

Currently 2 Consultant clinics per week. One clinic runs every week and is a single handed consultant clinic. One clinic runs alternate weeks and is supported by one resident doctor. One clinic runs alternative weeks and is a single handed consultant clinic.

### Downe Hospital

One general high risk consultant clinic, supported by a specialist doctor and one resident, higher trainee in Obstetrics.

Labour Ward

We provide 60 hour prospective consultant presence for labour ward by working in teams on dedicated days of the week. When on duty for labour ward, consultants will not be timetabled for other clinical duties between 08.30 and 16.30 hours.

Out of hours working

All out of hours work and on call will be for obstetrics. When on call, the consultant will be on call for the day from 08.30 to 18.30 hrs, resident in the hospital and from 18.30 to 08.30 the following day on call from outside the hospital (within a maximum commute of 30 minutes). Weekends will be resident in hospital 0900-1300 on a Saturday and 0900-1300 on a Sunday, there will be a resident 16:30 to 18:30 consultant lead ward round each weekend and on stat day with the remainder of the weekend hours being on call from outside the hospital.

Consultant will be expected to attend for all cases as laid out in RCOG Guidance “Roles and Responsibilities of an on call consultant”.

There is an on-site flat for the purposes of out of hours on call only, for those consultants who prefer to stay close by.

The new Consultant will participate pro-rata on a 1 in 12 on call rota

Other permanent staff and trainees

Permanent staff also include 6 Speciality Doctor posts, at present 4 are fulltime and 2 are part time. The consultants are supported at present by 7 WTE specialist trainees on the second tier rota. The first tier rota has a compliment of 7 ST1-2 doctors in obstetrics and gynaecology or general practice. 3 Fy2 doctors are on a separate rota. All trainee work a full shift rota pattern. We have 1 fulltime Physician Associate.

Fixed flexible sessions.

The Obstetric Consultant of the Day works a flexible fixed session to provide 08:30 to 16:30 Obstetric Consultant of the day. The Obstetric Consultant attends the 08:30 daily unit handover, conducts the antenatal and postnatal ward round, oversees the management of the wards, provides advice and attends EOU when required, reviews referrals from community midwifery service, provides advice and reviews women at the request of the sonographer conducting to anomaly scans and supports the labour ward consultant with complex cases.

Details of the Post

The holder of these posts will be expected to work within Maternity and Gynaecology Sub Directorate structure which is led by a Clinical Director. The holder will be directly responsible to the Associate Clinical Director of Obstetrics and Gynaecology. This will mean attendance at Sub Directorate Meetings and willingness to progress developments or quality initiatives agreed by the Directorate.

Fulfilment of continuing professional development (CPD) to meet appraisal and revalidation standards will be required. Practicing evidence-based medicine and effective team working within the multi-disciplinary team is essential. Participation in clinical audit and appropriate quality assurance exercises is a contractual requirement. The post-holder will be expected to attend, participate in and demonstrate leadership in ‘rolling’ medical audit meetings, morbidity and mortality review, and other internal and external review processes as deemed necessary by the Trust.

The post holder will be supported in their role by appropriate clerical and administrative staff and with dedicated office space and adequate equipment including IT facilities.

### Main Responsibilities

### The appointee will

share responsibility for inpatient, outpatient and day care services within the specialty of Obstetrics as described above;

communicate and liaise with local General Practitioners, other hospitals and other medical and paramedical personnel as required;

work within the Directorate as a member of the interdisciplinary team thus ensuring continuity of approach and care;

participate in the “On-Call” rota with the other Consultants. On call duties are currently performed from within the hospital for a stipulated 60 hours per week with the remainder from home, but may at some time in the future become resident;

report to the Clinical Director of the Sub Directorate for the maintenance of personal professional standards;

be expected in time to participate in the training, teaching and assessment of undergraduate medical students and postgraduate medical staff. If required the job plan will be reviewed and adjusted accordingly;

participate essential in medical and clinical audit and, where applicable, apply the results to improve service;

be actively involved in reviewing and updating clinical guidelines, policies and protocols relevant to their clinical practice

continue to develop knowledge, skills and expertise through Continuing Medical Education to which the Trust is committed and which is facilitated through study leave and funding;

undertake administrative duties associated with patient care to ensure arrangements are in place to meet specified quality standards;

comply with such requirements for revalidation as are determined by the General Medical Council;

undertake administrative duties within the Sub Directorate including attendance at audit, medical staff and management committees;

research will be encouraged provided it is organized through the correct channels within the Trust.

There will be an opportunity to develop a special interest conforming to the needs of the Obstetric service. If required the job plan will be reviewed and adjusted accordingly.

Patient Administration

This activity covers the management of individual patients including outpatient administration, results reporting, letters / phone calls to patients, carers, GPs and members of the multidisciplinary team involved in the patients care. The post holders will have appropriate secretarial support.

Teaching, Audit, Research & Quality Improvement

The Ulster Hospital is a major teaching hospital that receives a large number of medical students. It is affiliated with the Queen's University Belfast and the Ulster University. The successful candidate may in time play an active role in undergraduate and postgraduate teaching, during supporting programmed activities (SPA). Similarly, formalized supervision of resident doctors in time may be available, and would be implemented by NIMDTA via programme directors and college tutors. If roles incorporating teaching and or supervision of resident doctors are agreed with the post-holder the job plan will be reviewed and adjusted accordingly.

Participation in clinical audit together with quality improvement activities, contributes to medical appraisal and revalidation. The post holder will contribute to this programme and undertake their own audit projects. The Trust’s audit department can provide expertise on audit design, give practical support with data collection and advice on presentation of findings.

The Innovation, Research and Development (IRD) department offers a comprehensive service designed to support research activity within the Trust. The IRD office can assist with new research proposals by helping to secure approvals from regulatory authorities and providing support during the course of the project.

The Trust has established a Quality Improvement Academy to support staff with quality improvement and innovation work.

The post holder will be required to play an active role in medical and clinical audit, Morbidity and Mortality, SAI and other patient safety review processes.

Job Plan

The job plan below is indicative and subject to change. If required to work at a different site this will be factored into the PA allocation/ job plan of the successful applicant. It will also be subject to regular review (within 3 months in the first instance and no less than annually thereafter) to ensure that the allocation of PA’s, and in particular the split between DCC and SPA’s remains appropriate. Any changes will be made with agreement between the post holder and clinical management.

It is ideally located for urban, rural or semi-rural commuting. Belfast City centre is approximately 20 minutes away by public transport. George Best airport is 15 minutes away. The area has a large number of highly rated schools, attractive property buying opportunities and the area boasts many excellent sporting venues, such as Royal Belfast Golf Club, Ulster Rugby stadium and Royal North of Ireland Yacht Club.

Leisure opportunities are extensive with many excellent restaurants and other amenities available in close proximity to the hospital.

The Ulster Hospital

The Ulster Hospital is the major acute hospital for the Trust and delivers the full range of acute services for the population. This includes a type 1 Emergency Department which is open 24/7 (22/23 ~115k new & unplanned attendances), a comprehensive range of diagnostic services, the full range of outpatient, inpatient and daycase medical and surgical services, cancer care, coronary care, obstetrics, paediatric services and a psychiatric inpatient unit. The regional Plastic and Maxillo-Facial services are mainly delivered from the Ulster site.

The Ulster Hospital has undergone significant redevelopment with the building of a permanent renal unit, new maternity hospital, a new inpatient ward block and most recently an acute services ward block recently opened to add to, a new theatre block and Critical Care Complex that opened in November 2010.

The new inpatient ward block (IWB) was opened in February 2017 with 288 single en-suite rooms, 4 day surgery theatres and 3 endoscopy rooms.

The Acute Services Ward Block (ASB) is now operational. It includes 213 single en-suite rooms, a new X-ray department including CT, MRI and Nuclear Medicine imaging. The Emergency Department, which completes the final stage of the £280m Phase B redevelopment project, opened on the 6 September 2023.

The Ulster Hospital is a major teaching hospital associated with Queens University of Belfast and with the Ulster University. The library is linked to Queens University Medical Library and comprehensive on-line search facilities and other resources are provided through library connections. Facilities for Med-line and Cochrane Database are also available. Research opportunities are also being developed, particularly in the field of bronchiectasis, lung cancer and health related technology.

Lagan Valley Hospital

Lagan Valley Hospital is located in the city of Lisburn, a 30-minute commute from the Ulster Hospital. It is situated close the M1 motorway, with easy access from Belfast. The Emergency Department team operate an Urgent Care Centre, which is a phone first system (appointment only), which is open 8am-6pm Monday-Friday (closed at weekends).

The hospital has 84 inpatient medical beds, a psychiatric inpatient unit and a Dementia Assessment and Treatment Unit, as well as outpatient, diagnostic, day-case services for the local population. It also provides maternity services, including Consultant lead antenatal clinics and a range of outpatient gynaecology services including general gynaecology clinics, a recurrent pregnancy loss clinic, colposcopy clinics and outpatient hysteroscopy clinics. The Midwifery led birthing Centres are currently suspended and under review at Lagan Valley Hospital and Downe Hospital.

Downe Hospital

The Downe Hospital is located in the culturally historic town of Downpatrick, a 40-minute commute from the Ulster Hospital. The Emergency Department team operate an Urgent Care Centre which is open 8am-6pm Monday-Friday. There is also a Minor Injury Service on Saturday and Sunday 9am-5pm.

The hospital has 40 inpatient medical beds, a psychiatric inpatient unit and a Dementia Assessment and Treatment Unit. A Frail Elderly Rapid Assessment Clinic is staffed 9am to 6pm Monday to Friday. The Downe provides the Trust wide Bowel Screening Service and Sexual Health Service, as well as a range of outpatient, diagnostics and day-case services, such as cataract surgery. Two Primary Care practices are located on the first floor, along with community and dental services. There is a consultant led antenatal clinic and gynaecology outpatient and colposcopy clinic in this hospital.

Ards Hospital

Ards Community Hospital provides a comprehensive range of health, primary and social care services, including an X-Ray Department, GP Ward, Medical, Surgical, Gynaecology and Psychiatry Outpatients Department and GP Out of Hours services. The Trust’s breast screening service is delivered from a new building on the site.

Bangor Community Hospital

Bangor Community Hospital provides a range of health, social and primary care services, including an Outpatients Department, a Therapy & Rehabilitation Centre, and an X-Ray Department. Gynaecology outpatients and urogynaecology clinics are held on this site. The continence service for the area is based in Bangor Hospital and urodynamic studies are performed on an outpatients basis.

## Responsibilities

share responsibility for inpatient, outpatient and day care services within the specialty of Obstetrics as described above;

communicate and liaise with local General Practitioners, other hospitals and other medical and paramedical personnel as required;

work within the Directorate as a member of the interdisciplinary team thus ensuring continuity of approach and care;

participate in the “On-Call” rota with the other Consultants. On call duties are currently performed from within the hospital for a stipulated 60 hours per week with the remainder from home, but may at some time in the future become resident;

report to the Clinical Director of the Sub Directorate for the maintenance of personal professional standards;

be expected in time to participate in the training, teaching and assessment of undergraduate medical students and postgraduate medical staff. If required the job plan will be reviewed and adjusted accordingly;

participate essential in medical and clinical audit and, where applicable, apply the results to improve service;

be actively involved in reviewing and updating clinical guidelines, policies and protocols relevant to their clinical practice

continue to develop knowledge, skills and expertise through Continuing Medical Education to which the Trust is committed and which is facilitated through study leave and funding;

undertake administrative duties associated with patient care to ensure arrangements are in place to meet specified quality standards;

comply with such requirements for revalidation as are determined by the General Medical Council;

undertake administrative duties within the Sub Directorate including attendance at audit, medical staff and management committees;

research will be encouraged provided it is organized through the correct channels within the Trust.

There will be an opportunity to develop a special interest conforming to the needs of the Obstetric service. If required the job plan will be reviewed and adjusted accordingly.

Patient Administration

This activity covers the management of individual patients including outpatient administration, results reporting, letters / phone calls to patients, carers, GPs and members of the multidisciplinary team involved in the patients care. The post holders will have appropriate secretarial support.

Teaching, Audit, Research & Quality Improvement

The Ulster Hospital is a major teaching hospital that receives a large number of medical students. It is affiliated with the Queen's University Belfast and the Ulster University. The successful candidate may in time play an active role in undergraduate and postgraduate teaching, during supporting programmed activities (SPA). Similarly, formalized supervision of resident doctors in time may be available, and would be implemented by NIMDTA via programme directors and college tutors. If roles incorporating teaching and or supervision of resident doctors are agreed with the post-holder the job plan will be reviewed and adjusted accordingly.

Participation in clinical audit together with quality improvement activities, contributes to medical appraisal and revalidation. The post holder will contribute to this programme and undertake their own audit projects. The Trust’s audit department can provide expertise on audit design, give practical support with data collection and advice on presentation of findings.

The Innovation, Research and Development (IRD) department offers a comprehensive service designed to support research activity within the Trust. The IRD office can assist with new research proposals by helping to secure approvals from regulatory authorities and providing support during the course of the project.

The Trust has established a Quality Improvement Academy to support staff with quality improvement and innovation work.

The post holder will be required to play an active role in medical and clinical audit, Morbidity and Mortality, SAI and other patient safety review processes.

Job Plan

The job plan below is indicative and subject to change. If required to work at a different site this will be factored into the PA allocation/ job plan of the successful applicant. It will also be subject to regular review (within 3 months in the first instance and no less than annually thereafter) to ensure that the allocation of PA’s, and in particular the split between DCC and SPA’s remains appropriate. Any changes will be made with agreement between the post holder and clinical management.
