### Consultant in Palliative Medicine

### 6.5 PA permanent

We are seeking a substantive community consultant in Palliative Medicine for the South Camden and Islington Community Palliative Care Team.

This post sits within the Camden, Islington and UCLH  Palliative Care Service. Doctors on the specialist register for SPC or due to CCT within 6 months are welcome to apply. There are on call duties aligned to this post.

### Main duties of the job

Medical consultants within the Service work primarily within the South Camden and Islington community palliative care service, and in addition to clinical care,  each are  closely involved with strategic development and service improvement, as well as the advancement of relationships with key colleagues. The aims of the post are to provide an efficient and responsive clinical service to the community patients requiring supportive and palliative care; the post holder will join a team of 3 community consultants ( 2 medical, 1 nursing) to develop relationships and improve the care of patients and provide cross clinical cover within other areas of the specialist palliative care service (including the UCLH hospital palliative care team). This is a substantive consultant role of 6.5 PAs. All DCC are within the community setting with on call across both community and hospital.

### Detailed job description and main responsibilities

- Provide clinical care to patients and those important to them within South Camden, and Islington (small part of Westminster). This includes visiting patients alone or jointly in their place of residence, providing telephone advice to other team members and other professionals, and conducting regular caseload reviews with the Clinical Nurse/Paramedic Specialists.  The post holder will work closely all colleagues in the team, as well as colleagues within the wider service and throughout the Trust, and will liaise with professionals within the community - GPs, Community Nursing and other specialist Teams (e.g. Respiratory, Learning difficulties, Heart Failure, Dementia etc), and within St Pancras Hospital (St Pancras Rehabilitation Unit, Camden Virtual ward etc) as well as neighbouring hospitals  as needed.
- Provide clinical cross cover for SPC consultants in the service who work in UCLH as needed.
- Work closely with the clinical lead for the service Dr Rasha Al-Qurainy, as well as the leads for the community team.
- Represent Palliative Care at relevant clinical and strategic meetings (e.g. NCL Specialist Palliative Care provider group, PallE8, GP Multidisciplinary meetings, Camden cross community meetings)
- Attend and contribute to the community specialist palliative care MDT meeting each week.
- Provide clinical supervision and educational supervision to trainees working within the team, as well as educational support and teaching to members of the nursing team undertaking advanced practice (e.g. non-medical prescribing, physical assessment) and to visitors to the team.

- An understanding of the principles and practical applications of clinical governance systems, education, quality improvement

- Ability and commitment to teach in a clinical and academic setting

- Ability and commitment to teaching various clinical disciplines

### Education, Qualifications and Training

**Essential**

- Primary Medical Degree
- Approved Clinician status
- Full GMC Registration with a Licence to practice
- Entry on the GMC Specialist Register in Palliative Medicine through CCT, Portfolio Pathway (CESR), REQ, or RSQ routes.

**Desirable**

- An additional postgraduate qualification of relevance to the work
