### Job overview

This is the first substantive,combined Consultant post in Genitourinary Medicine and General Medicine in GSTT. The post holder will provide general and complex GUM including LARC and a specialist area to be defined, and 8 weeks of internal medicine cover per year divided between ward cover and the acute assessment unit.

### Main duties of the job

This is a part-time (8 programmed activities/0.8 Whole time equivalent)  . The post holder will provide complex sexual health care, leadership within the sexual and reproductive health department, training, as well as general clinical and managerial responsibilities in the department.

The post holder will contribute to clinical, teaching, audit and research support for the department and management support for the medical staff and the department in general. The service currently operates a senior doctor model where the named senior doctor provides oversight and support for the service as well as managing referrals and hospital/primary care queries.

The post holder will provide leadership for a specialist area of GUM to be negotiated within the consultant team.  There is potential to develop other areas of interest as the department requires.

The post includes a regular General Internal Medicine commitment, delivered in discrete blocks across the year. This includes weekend and out of hours commitments and comprises:

- Four separate 1-week Acute Admission Ward/acute medicine blocks
- One 4-week block on the North Wing general medical ward.

- Duties and Responsibilities

Clinical:

The post holder will, together with consultant colleagues, be responsible for the provision of Sexual and Reproductive healthcare services to the Guy’s & St Thomas’ NHS Foundation Trust to include:

- Complex sexual health services
- Specialist Genitourinary medicine clinics

Consultant referral and review clinic

- Advice and guidance for Sexual health:

Provide High Quality Care to Patients

- The post holder must be medically qualified and maintain GMC specialist registration and hold a licence to practice
- To develop and maintain the competencies required to carry out the duties required of the post.
- To ensure prompt attendance at agreed direct clinical care Programmed Activities.
- To ensure patients are involved in decisions about their care and to respond to their views.

Research, Teaching and Training

- Where possible to collaborate with academic and clinical colleagues to enhance the Trust’s research and academic training portfolio, at all times meeting the full requirements of Research Governance.
- To provide high quality teaching to medical undergraduates and members of other health care professions as required by the Clinical Lead.
- To act as educational supervisor and appraiser as delegated by the Clinical Lead to ensure external accreditation of training post.

Performance Management

- To work with medical, nursing and managerial colleagues to ensure high performance in the following areas:

Clinical efficiency e.g. optimising clinic capacity with demand and reducing DNA rates.
Quality of outcomes e.g. infection control targets, reducing rebook rates.
Financial management e.g. identification, implementation and achievement of cost improvement programmes and participating in efforts to ensure services are provided cost effectively e.g. managing locum agency spend, ensuring clinical teams code activity accurately so tariffs are not missed
Operational efficiency e.g. waiting list activity and demand management.

- To participate in the recruitment of junior medical staff as and when required.
- To participate in team objective setting as part of the annual job planning cycle.
- To be responsible for the annual appraisal of all doctors in training, Trust doctors, Clinical Fellows and non-consultant grades as delegated by the Clinical Lead/General Manager

Strategy and Business Planning

- To participate in the business planning and objective setting process for the directorate and Trust where appropriate.
- To represent the Trust at appropriate clinical networks/other external clinical meetings, as delegated by the Clinical Lead.

Leadership and Team Working

- To demonstrate excellent leadership skills with regard to individual performance, clinical teams, the Trust and when participating in national or local initiatives.
- To work collaboratively with all members of the multi-disciplinary team and Kings Health Partners as required.
- To chair regular meetings for the specialties.
- To resolve conflict and difficult situations through negotiation and discussion, involving appropriate parties.
- Adhere to Trust/departmental guidelines on leave including reporting absence.

- The indicative Job Plan

The job plan is anticipated to contain 8 Programmed Activities per week on average to be carried out on Guy’s & St Thomas’ sites. It includes 1 in 12 Saturdays for GUM as well as weekend and out of hours commitments in the medicine blocks.  The timetable will be flexible and change between GUM, Acute medicine and General Medicine blocks.

This is an outline programme and the post holder will be expected to discuss and agree a detailed job plan including personal and professional objectives with the clinical lead within 3 months of your start date.

The job plan will be negotiated between the consultant and their clinical lead at least annually. The initial job plan for this post is planned to be:

General medicine component – these weeks are worked full time.

General Internal Medicine commitment, delivered in discrete blocks across the year. This comprises:

- Four separate 1-week Acute Admission Ward/acute medicine blocks.
- One 4-week block on the North Wing general medical ward.

During the AAW/acute medicine weeks, Start times vary between 08:00 and 09:00, with daytime availability into the afternoon, and additional out-of-hours commitments including twilight shifts from 17:00–21:00, weekend working, day take and night on-call depending on the rota week. These weeks include both routine ward-based activity and acute unselected take responsibilities.

The Ward commitment is delivered as a 4-week block as consultant on a general medical ward.. The usual weekday commitment is approximately 09:00–17:00, with some 08:00 ward round starts depending on the day. The North Wing rota also includes weekend ward cover on a 1 in 4 basis.
