From our award-winning frailty model and dedicated Frailty Take service, through to community rehabilitation, Parkinson's disease services, stroke care and integrated multidisciplinary working, we are creating seamless pathways that deliver the right care, in the right place, at the right time. Experienced in Clinical Audit and Clinical Governance and the benefits of how this improves the quality of care provided to patients and ensuring this is embedded in clinical practice. ### Qualifications **Essential** - GMC Registration with a Licence to Practice.
