### Job overview

This is an exciting  opportunity to a join a multidisciplinary team of specialised health professionals dedicated to the Aintree site complex polyp service. This role is torun the complex polyp service with support from a Nurse Endoscopist service lead. Complex polyp experience is essential.

The complex polyp service sits under the Gastroenterology Clinical Nurse Specialist team, there are close links with the surgical CNS and BCSP teams.

Complex polyp Clinical Nurse Specialist will be expected to undertake a broad range of highly skilled, patient facing snf pathway critical responsibilities including:

Management of patients requiring high risk complex endoscopic proceudres

Autonomous nurse led clinics, including new patient assessmen, histology, site check and surveillance clinics.

Reviewing complex polyp referrals, listing for complex polyp MDT and referring onto other MDT's where appropriate.

Leading the complex polyp MDT and acting upon the outcomes, commmunicating these to patient / referring team.

Attend SERC / SPECC MDT and act upon the outcomes of any non-surgical patients.

Patient communication and support.

The main duties of the role will include:

Perform specialist assessment of patients nursing needs, plan, implement and evaluate care delivery using advanced clinical knowledge and clinical skills. manage clinical caseload of patients providing expert assessment, planning and evaluation Assist in the running of nurse-led clinics (within Speciality). Contribute towards a multi-professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements. Establish and maintain effective communication with patients and carers / relatives. Support patients and their families and ensure that they receive the required information to enable them to self manage, lead or participate in their care delivery. Recognise changes in patients condition that require the intervention of others and refer on as appropriate. To support, assist and encourage the multi-disciplinary team in resolving patient problems, by the provision of new and innovative models of case management. To assist in the delivery of high standards of patient care. To work in partnership with lead clinicians, General Managers, Matron to ensure the delivery of high standards of patient care.
