Working independently and under direction and support of a registered clinical practitioner, the CSW will visit people at home who are experiencing an urgent health and social care crisis. The CSW helping the person in their immediate recovery and enabling them to remain at home or place of residence (where safe to do so) without the need for an unnecessary hospital conveyance. The CTS role is ideally suited for someone who is compassionate, resilient and motivated to make a real difference. Community urgent same day care services operate county wide and the role will be working shift patterns and occasional weekends between 8am and 8pm, 365 days a year. The post-holder may be required to work flexibly across shift patterns and will operate within a locality area, however the services are county wide and some travel will be required.
