The West Division of Betsi Cadwaladr University Health Board delivers District Nursing Services across the Counties of Anglesey and Gwynedd working in an integrated way with partner organisations to deliver high quality evidence based care in order to maintain patients in their own homes and communities, averting hospital admissions, empowering the patient towards self management and implementing seamless patient centred care and treatment. You must be able to travel between patients homes both within towns and rural areas in a timely manner.
