\<p\>As a Band 5 Community Staff Nurse, you will join our valued Swanley and Rural PCN Community Nursing Team in North Kent. This is a rewarding opportunity to deliver high-quality nursing care where it matters most, within patients\\' own homes and local communities. You will \<span class="single_desc_int_gb_word"\>support people with a wide range of healthcare needs, helping them manage long-term conditions, recover from illness and maintain their independence while avoiding unnecessary hospital admissions\</span\>.
