Every day brings variety, autonomy, and the privilege of building genuine therapeutic relationships with the people you serve.

As a Band 5 Community Nurse in the beautiful rural locality of South West Shropshire, you’ll deliver compassionate, high‑quality care to people in their own homes, bringing autonomy, variety, and real purpose to your day.

What You’ll Be Doing:

- Delivering holistic nursing care that helps patients stay at home, avoid unnecessary hospital admissions, and achieve safe, timely discharge.
- Working as an autonomous practitioner across a diverse and rewarding caseload, providing key clinical interventions.

Hours will be worked over a 7‑day service, 8am–6pm. Flexible working options available.

## Person Specification

### Experience

**Essential**

- Post registration general experience
- Experience of working in a multi disciplinary / multi agency health/social care environment.
- Experience and knowledge of basic administration systems including Microsoft office
- Experience of successful partnership working arrangements with other services
- Previous community experience

### Qualifications

**Essential**

- RGN
- NMC Registered
- Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes

### Knowledge and Skills

**Essential**

- IT skills or willingness to undertake RIO Training
- Up to date with Mandatory Training
- Uses evidence based practice
- Works within own defined competencies
- Awareness of changing healthcare factors influencing change Demonstrates tact and diplomacy
- Well refined communication (verbal and written), interpersonal skills – ability to gain rapport at all levels
- Engaging and empowering approach that seeks to work with others and deliver outcomes in a successful and well informed manner
