We aim to deliver high quality care to improve health & wellbeing and to support our patients to remain living at home. We pride ourselves in being hard-working, supportive, friendly and flexible. We are part of the North & West Primary Care Network and work closely with partners across the network. We are based in the market town of Kington nestled within the beautiful County of Herefordshire - covering both Kington and Weobley surgeries in the north-west area of the county. This rural area has many areas of outstanding beauty and is predominantly agricultural with rolling country side for miles. We're seeking to recruit a band 5 community nurse who shares our passion for caring for people in their own homes, strives towards excellence in community nursing and who has the ability to work well within a close knit team of supportive colleagues. Our working shifts are 08:00 to 16:00 and 12:00 to 20:00, over a 7 day period, including regular weekends. There will be an expectation to work within the North West Locality, though the base for this job is Kington. ### Main duties of the job Our aim is to provide treatment and support to patients in their own homes. Whilst undertaking a holistic nursing assessment, we aim to encourage patients to remain as independent as possible, in order to prevent hospital admissions. Due to the nature of this role, the ability to meet the travel requirements of the post are essential.
