We have an exciting opportunity for the right candidate to join our wonderful District Nursing Team. The Golden Valley District Nursing Team are a small but very supportive and dedicated team, we are an incredibly engaging, positive and hard-working team, providing care and support to patients in their own homes. Due to the small size of the team with a very large geographical area to cover, community experience/knowledge is beneficial and advised for this post. We work closely with a wide range of multi-disciplinary teams, including GP's, Palliative Care Nurses, Therapy Teams, and Advanced Clinical Practitioners. We welcome applications from both experienced and newly qualified nurses (beneficial to have Community knowledge). ### Main duties of the job Our aim is to provide treatment and support to patients in their own homes. Whilst undertaking a holistic nursing assessment, we aim to encourage patients to remain as independent as possible, in order to prevent hospital admissions. Due to the nature of this role, the ability to meet the travel requirements of the post are essential. The Golden Valley cover one of the largest areas within the service, this post will include a high amount of travel.
