We have an exciting opportunity for two enthusiastic and highly motivated Mental Health Nurses to join the Community Mental Health Rehabilitation Team (CMHRT). The team is temporarily based at Engineers House on the St Austell Hospital site; however, as we deliver a countywide service, applications are welcomed from across Cornwall.

We are seeking individuals who can demonstrate the Trust’s vision and values in practice and who are passionate about working with people living with complex psychosis. This role offers the opportunity to make a real difference to the lives of service users and their families by delivering high-quality, person-centred care.

The CMHRT provides care, treatment and rehabilitation interventions, focusing on strengths, promoting hope, and building self-esteem and confidence. Our aim is to support service users to achieve their optimum level of functioning, live well in the community, and manage periods of crisis.

This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.

### Main duties of the job

Work effectively both as a member of the multidisciplinary team and independently as a lone worker, in line with Trust policies.

Provide supervision and support to students and other learners within the team, ensuring practice is compliant with Trust and professional standards.

Maintain an up-to-date professional portfolio, including mandatory training, annual appraisal documentation, and supervision records.

Act as a mentor for student university placements, holding a recognised mentoring qualification or demonstrating willingness to work towards this and attend mentorship updates as required.

Deliver specialist and generic clinical interventions to enhance engagement with services, supporting service users and carers to access treatment that promotes wellbeing and recovery.

Undertake comprehensive assessments and develop holistic, person-centred care plans incorporating positive risk management and evidence-based interventions.

Ensure service users and carers are actively involved in assessment, formulation and care planning, demonstrating awareness of cultural and diversity factors and addressing barriers to accessing care.

Communicate effectively with professionals across disciplines and services, using a whole-team approach, and support service users to clearly and confidently express their needs.

Build and maintain therapeutic relationships that promote engagement and ongoing adherence to agreed care packages.

## Person Specification

### Experience

**Essential**

- Working knowledge of crisis assessment and intervention

**Desirable**

- Previous work with community/ Acute services
- Previous experience within housing employment services

### Education / Qualifications and Relevant Experience

**Essential**

- Current professional registration – NMC
- Professional qualification – RMN,

**Desirable**

- To act as a mentor for student university placements. Having the mentoring qualification or being willing to work towards this qualification and attending mentorship updates as required
