We are looking for a Community Staff Nurse to join our friendly and supportive Community Nursing Team. This is an excellent opportunity to develop your community nursing skills, work closely with a wide range of health and social care professionals and contribute to the ongoing development of services that make a positive impact on people's lives. ### Main duties of the job As a Community Staff Nurse, you will: - Deliver high-quality, person-centred nursing care to patients in their own homes and community settings. - Assess, plan, implement and evaluate care to meet individual patient needs. - Work in partnership with patients, carers and multidisciplinary colleagues to coordinate effective care and support. - Promote the highest standards of care, compassion and professionalism in everything you do. - Support the coordination of the service during the short-term absence of the District Nurse. - Work closely with GPs, practice teams, palliative care services, social care teams and voluntary agencies to ensure seamless patient care. - Support and encourage colleagues, learners and support workers within the team. - Contribute to service development, quality improvement initiatives and the advancement of community nursing practice. - Maintain and develop your clinical knowledge and skills through ongoing professional development.
