Bolton Community Mental Health Services are a committed integrated service that strives to provide quality recovery based support to people with severe and enduring mental health related difficulties in Bolton 7 days per week, 365 days per year. The service calls upon a wide range of evidence based holistic assessments and interventions. As a Community Psychiatric Nurse working in the service you will be required to undertake the role of Care Coordinator in line with CPA process, you will have a defined complex caseload which could consist of people who are on the Assertive Outreach pathway, and will be expected to work proactively and flexibly within a multi-disciplinary team model, and you will be expected to work outside normal office working hours.  You will also be expected to act as assessor/mentor for student nurses.  This job description is not exhaustive, but is intended to give an overall picture of the role. Other duties within the general scope of the post may be required from time to time. The duties of the post and job description can be reviewed through the agreed process

### Main duties of the job

To provide skilled mental health nursing care to people in the community. To assume the role of care coordinator for a complex caseload. To work as a member of a multi-disciplinary team. To have a theoretical and working knowledge of the Safeguarding Adults and Children’s framework, and to take the lead role when necessary. To undertake initial assessments of people referred into the service, linking in directly with the Team Leader and wider MDT. To devleop implement and evaluate holistic programmes of care, including the assessment of carer’s needs and assessments.   To maintain close links with other statutory and non-statutory professionals and agencies, and to be able to access resources from provider services, and to maintain good channels of communication. To undertake the role of care coordinator in accordance with the philosophy of the Care Programme Approach and associated Mental Health Act (1983) legislation and Care Act legislation.

## Person Specification

### Essential

**Essential**

- Registered Mental Health Nurse – Diploma in Mental Health Nursing or Degree/BSc in Mental Health Nursing.

### Knowledge

**Essential**

- Continued professional development in accordance with the requirements of the Nursing and Midwifery Council
- Demonstrates ability to use initiative to deal with situations which may occur
- Ability to work collaboratively with colleagues and those from other disciplines
- Have good verbal and written communication skills

### Experience

**Essential**

- It is essential that the post holder has demonstrable post qualifying experience

**Desirable**

- Experience of working within a community setting

### Skills and Abilities

**Essential**

- To demonstrate up to date clinical & professional practice and expertise via the delivery of appropriate interventions.
- Demonstrate the ability to assess mental health needs and the risk profiles of service users and the ability to plan, deliver and evaluate care and treatment based on the outcome of assessments using Trust and Local policies and procedures in relation to the Care Programme Approach (CPA).

**Desirable**

- To have undertaken work within a community based mental health team (CMHT).
