The service calls upon a wide range of evidence based holistic assessments and interventions. As a Community Psychiatric Nurse working in the service you will be required to undertake the role of Care Coordinator in line with CPA process, you will have a defined complex caseload which could consist of people who are on the Assertive Outreach pathway, and will be expected to work proactively and flexibly within a multi-disciplinary team model, and you will be expected to work outside normal office working hours.  You will also be expected to act as assessor/mentor for student nurses.  This job description is not exhaustive, but is intended to give an overall picture of the role. To assume the role of care coordinator for a complex caseload. To work as a member of a multi-disciplinary team. To undertake initial assessments of people referred into the service, linking in directly with the Team Leader and wider MDT. To devleop implement and evaluate holistic programmes of care, including the assessment of carer’s needs and assessments.   To maintain close links with other statutory and non-statutory professionals and agencies, and to be able to access resources from provider services, and to maintain good channels of communication.
