### Job summary

We are looking for enthusiastic, experienced, innovative mental health nurses who are committed to the principles of recovery. We are a small, friendly team who believe in working together and supporting each other in managing complex cases. We are based in the scenic area of the New Forest and are situated on the Hampshire and Dorset boarder, so would welcome applications from those living outside of Hampshire or who are looking to relocate.

We believe in delivering specific episodes of care to empower people to work towards recovery.

Due to the current skill mix in the team we are only open to male applicants in order to ensure we maintain a sufficient skill mix for service delivery. We have a well established multi disciplinary team including, team manager, team leader, consultant psychiatrists, psychologists, Independent Nurse Prescribers, RMN's, support time recovery workers, carers officers and employment support.

We are happy to consider flexible working requests and remote working where possible. We are operational Monday to Friday.

We believe in supporting our staff to develop in the careers through training and you will receive regular clinical and management supervision. We also run monthly reflective practice, education slots and safeguarding supervision.

### Main duties of the job

The role of the care coordinator is to have an agreed caseload, working collaboratively with service users and carers. There is a wide range of interventions that can be offered which may include psychological interventions, medication support, access to group work and carers support. We aim to engage service users creatively, develop collaborative care plans including risk management plans, care plans and crisis contingency plans. You will have access to the MDT and a leadership team to receive support and guidance when required.

You will need robust assessment skills to enable the team to deliver high quality care and treatment plans. Excellent communication skills are also essential to enable you to be a role model for others, as well as positively representing the team and the organisation as a whole.

We are looking for those with good communications skills and work well in a multi disciplinary team. We are keen for staff to take leadership roles where appropriate and to provide supervision to other members of the team.

You must also be able to liaise effectively with other services, both internal and external to HIOW Health Foundation NHS Trust, retaining service user focus at the heart of any clinical decision making to ensure the best outcome.

### Qualifications

**Essential**

- Registered Nurse, Mental Health Practitioner.
- Recognised Mentorship qualification.
- 24 months post registration experience.
- Able to attend and participate in all statutory & mandatory training events to enhance continued professional development
- Willingness to complete further studies as necessary to meet service delivery requirements.
