HERE WE ARE!

We are looking for committed and enthusiastic nurses to join our Cognitive Impairment and Dementia Specialism (CIDS) which provides community-based treatment and care to all adults developing cognitive deficits and those with dementia. The role of the Community Nurse is to work in partnership with patients and their families/carers to enable them to lead fulfilling lives.

To provide specialist assessments, reviews and nursing interventions to patients accessing CIDS. The post holder will demonstrate an understanding of clinical interventions relevant to the care group, to include education, care plans, risk management and discharge planning, working closely with families/carers and partner agencies to facilitate this.  The post holder will always work to promote the safety and the wellbeing of patients and their families/carers.

COMMUNITY NURSE ROLE BAND 6 - HOUNSLOW

Salary                                    £45,952.85 - £54,254.00 per annum including 15% HCAS

### Main duties of the job

- The post holder will demonstrate an understanding of clinical assessments and interventions relevant to dementia care and will always work to promote the safety and the wellbeing of service users and their families/carers.
- Candidates will need to demonstrate understanding of the needs of older people living with dementia and physical health care challenges.
- Your role within the team would be to lead the delivery of care including Cognitive assessments, education, care plans and discharge planning, for patients with potential dementia and complex health needs,.
- You will provide clinical supervision to staff and students.
- You will receive superb training and development opportunities.  Working with this client group can be challenging and requires someone who is able to employ reflection and evidence-based learning to enhance their own and the wider team’s knowledge and skills.
- You will receive monthly supervision to support your development and practice

### Knowledge

**Essential**

- A high level of working knowledge of cognitive impairment and dementia.
- Ability to work within a Multi-Disciplinary Community Team.
- Deputise for the duty manager as appropriate.
- Have an excellent understanding of assessment and management of risk.
- Effective planning & organisational skills with an ability to prioritise own workload.
- Effective communications skills to meet the needs of the post in full.
- Effective management and leadership skills.
- Ability to identify solutions to problems and implement them effectively.
- Ability to work to tight timescales whilst meeting targets
- Demonstrate a commitment to the provision of high quality and safe services with an ability to drive a culture of continuous improvement.

### Experience

**Essential**

- Experience of working with patients and their carers with complex health and social care difficulties in the designated care group.
- Experience of supervising staff.
- Experience of managing clinical caseload will include carrying out initial assessments, treatment efficacy reviews and risk mitigation assessments leading to crisis prevention.
- Experience of providing a range of clinical interventions to people in the designated care group with a variety of health difficulties.
- Experience of working consultatively with professionals.
- Experience of conducting a range of clinical assessments including risk assessments.
- Experience of partnership working within and across various statutory and non-statutory teams and agencies.
- Previous experience of working with carers and independent / voluntary sectors.

**Desirable**

- Experience in working with people with dementia on the ward and in the community.

### Qualifications and Training

**Essential**

- Professionally qualified and registered Nurse with current NMC registration in Mental Health / LD/ Adult Nursing.
- Evidence of continuous professional development.
