The team supports individuals aged 65 and over who are experiencing a range of mental health conditions, including dementia, schizophrenia, bipolar disorder, personality disorder, anxiety, and depression. IHTT aims to give people the choice and ability to live as independently as possible, based on the belief that individuals experiencing mental health difficulties are entitled to the highest quality of care. Our service is delivered in people’s own homes, with a strong focus on preventing unnecessary admission to hospital or residential care. We provide intensive support for up to 8 weeks in the patient’s home, helping to optimise mental health, promote recovery, and maintain independence. As part of our dynamic and fast-paced multi-disciplinary team, you will play a key role in delivering high-quality, person-centred care while contributing to innovative ways of working. Our team brings together a wide range of professionals, including Consultants, Pharmacists, Psychologists, CBT Therapists, Nurses, Occupational Therapists, and Support Workers, who collaborate to provide holistic and specialist care for older people with complex, high-risk mental health needs. ### Main duties of the job You will act as a care coordinator, managing a caseload of patients and providing high-quality, person-centred care. Your role will include supporting the team’s daily duty function, responding to patient and carer enquiries, and conducting initial assessments for new referrals into the service. The Intensive Home Treatment Team operates 7 days a week, from 8:00 AM to 8:00 PM, including weekends and bank holidays, and you will be required to work flexibly within these hours as part of the rota. You will work collaboratively within a multi-disciplinary team, contributing to care planning and clinical discussions to ensure safe and effective care.
