The position will be covering the Mid Nottinghamshire area (Mansfield, Ashfield, Newark & Sherwood) . The successful candidate will work within the Mid Notts Community Therapy Team and support the delivery of Community rehabilitation services and Discharge to Assess. Our passion is to maintain independence and keep individuals at home for as long as they are able, avoiding hospital admission and enabling a return home where admission has been unavoidable. Working with us will broaden your skill sets and give you the scope and confidence to take your professional development wherever you want it to go! ### Main duties of the job Working within the integrated team, you will be supported to effectively triage, prioritise, assess and provide holistic interventions for clients referred to the service, within their home environment. You will work closely with health and social care colleagues across primary and secondary care to ensure continuity, effective intervention and maximise rehabilitation outcomes. You will offer bespoke physiotherapy to adults, the frail and elderly including complex fallers, patients with neurological conditions, complex co-morbidities and orthopaedics. We deliver services between 8am and 10pm over 7 days. We currently operate shift patterns for Band 5 posts between 8 am and 6pm on 5/7 days. ### Detailed job description and main responsibilities Applicants should hold BSc/Diploma in Physiotherapy, HCPC Registration. You will ideally have post qualification experience and recent experience of working with clients with long term conditions, falls, neurology and health care of the elderly conditions. A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010. Allied Health Professionals within Nottinghamshire Healthcare Foundation Trust are at the forefront of a range of multi-disciplinary, specialist services for people with complex needs, using their expertise around promoting recovery, enablement and positive risk taking, to add life to years, prevent relapse and reduce risks. ### Applicant requirements You must have appropriate UK professional registration.
