### Job overview

We are seeking a highly skilled and motivated Physiotherapist to join our team, delivering assessment and treatment to patients within the community, including their own homes and care settings across North Cornwall. You will work closely with your colleagues in the Integrated Neighbourhood Teams to support patients’ independence, recovery, and wellbeing.

This post is 0.5 Homefirst and 0.5 Community Rehabilitation Team. The vision is that this new post will improve patient experience and continuity of care by the same Physiotherapist being able to see the patient through their episode of care. The teams are based in the same office at Carew House and work closely together.

Key responsibilities include:

- Providing specialist physiotherapist assessment and interventions tailored to patients’ individual needs.
- Working collaboratively within a multidisciplinary team to ensure effective and timely care.
- Supervising and supporting junior staff and contributing to their development within the service.

This is an excellent opportunity to make a meaningful difference within a supportive and integrated community team.

### Main duties of the job

- To manage a defined caseload of patients with complex needs within the Community setting using evidence-based practice, physiotherapy process and patient-centered principles
- To provide a high standard physiotherapy to the service, performing advanced assessments of patients including those with highly complex presentations, and from this formulating a diagnosis and treatment plan.
- To operate as a practitioner being the first contact for urgent/soon requests to help both prevent hospital admission and facilitate early discharge.
- To have an active role with the triage of new referrals coming into the teams.
- To provide leadership and support for junior staff through supervision, training, and appraisal as well as leading with physiotherapy students
- To participate in the planning and development of physiotherapy services.
- To cover a 7-day Rota.
- Model and deliver excellent evidence based clinical care, working as an autonomous Practitioner.
- To support the community rehabilitation and re-ablement model in line with a ‘one workforce’ culture across services, settings, and sectors in the community and step-down intermediate care settings.

## Person Specification

### Experience

**Essential**

- Broad range of post graduate experience of medical, surgical, orthopedic, and neurological settings
- Extensive experience of working within a multidisciplinary team
- Comprehensive experience of working as a physiotherapist with patients with complex conditions
- Experience of managing the risks associated with managing patients unsupervised and in a lone working environment

**Desirable**

- A broad range of clinical experience in a variety of community and inpatient settings.
- Participation in clinical audit/research activity

### Skills and Aptitude

**Essential**

- High level of specialist assessment and treatment skills.
- Commitment to and understanding of teams and teamwork.

### Education and Qualifications

**Essential**

- Professional Qualification - Degree or Diploma in Physiotherapy.
- Current Professional registration - HCPC

**Desirable**

- Evidence of further training relevant to specialty e.g., Post Graduate Courses
