An exciting opportunity has arisen for an experienced and motivated Community Nurse to join our Care Home Team as a Band 6 Deputy Team Lead.

Based at Stanmore (HA7 1AT), the Care Home Team delivers high quality nursing care to residents across care homes within Hertsmere, St Albans and Harpenden, Dacorum, and Watford.

You will be supported by an experienced Band 7 Team Lead, Practice Development Nurses, and a range of specialist link nurses, with access to excellent learning and development opportunities through the CLCH Academy.

This role is ideal for an experienced community nurse looking to develop their leadership skills within a supportive and forward thinking team. The post holder will be required to work across the locality in response to service needs, including participation in a 1 in 4 weekend rota. As travel between care homes and community settings is required, a full UK driving licence and access to a vehicle for work purposes are essential.

### Main duties of the job

- To set and maintain standards of care and deploy, co-ordinate and supervise the nursing team and learners as required.
- As a Deputy Team Lead, you will be responsible for managing a caseload of patients within residential care settings, providing clinical leadership and supporting the day to day management of the team
- You will line manage and support Band 5, 4, and 3 staff, helping to develop confident, autonomous practitioners whilst ensuring the delivery of safe, effective, and compassionate care.
- To provide leadership and management to a team of community nurses and other staff covering the four localities across Herts Valleys.
- To be responsible for the team’s assessment, planning, implementation and evaluation of individualised nursing care to patients living in a residential home. The service is provided seven days per week 8pm-8am.
- To be responsible for the team’s maintenance of patient and other records.
- To participate in the long term management of patients within the locality.
- To have continued responsibility for a defined caseload.

- Knowledge of Evidence based practice
- Evidence of personal development
- Knowledge of NMC code of professional conduct

- First Level Degree

- Course of Top to Toe examination/ diagnosis Experience
