In this role, you will deliver high-quality, person-centred care to patients within the community, helping them to manage long term conditions, recover from illness and live independently in their own home. As a community nurse you will work closely with a supportive multidisciplinary team, ensuring patients receive the best possible care in the comfort of their own environment. What you will be doing: Delivering a range of nursing interventions, including wound care, chronic disease management, medication administration and palliative care. Care Planning: Working alongside the DN/Senior Community Nurse to develop, implement and review care plans for patients. Multidisciplinary Collaboration: Work as part of a team with doctors, care management and other healthcare professionals to ensure holistic, patient focused care. Clinical Skills: Undertake clinical tasks such as injections, dressings and monitoring of vital signs. We are looking for: - a registered nurse with a current NMC registration - A genuine passion for providing care in the community setting and supporting patients to live independently. - Strong clinical skills, particularly in areas such as wound care, medication administration and palliative care. - Excellent communication skills and the ability to build relationships with patients, families and team members. - Ability to work independently and manage your own caseload. - Full driving licence and own car.
