A position has arisen for a Band 5 Community Nurse within K&M District Nursing Team – primary base will be Inverbervie.

Are you a passionate and dedicated registered nurse looking to make a real difference to the lives of patients in their own homes and local communities? We are seeking an enthusiastic and skilled nurse to join our dynamic community healthcare team.

In this role, you will deliver high-quality, person-centred care to patients within the community, helping them to manage long term conditions, recover from illness and live independently in their own home. As a community nurse you will work closely with a supportive multidisciplinary team, ensuring patients receive the best possible care in the comfort of their own environment.

What you will be doing: Delivering a range of nursing interventions, including wound care, chronic disease management, medication administration and palliative care.

Care Planning: Working alongside the DN/Senior Community Nurse to develop, implement and review care plans for patients.

Multidisciplinary Collaboration: Work as part of a team with doctors, care management and other healthcare professionals to ensure holistic, patient focused care.

Clinical Skills: Undertake clinical tasks such as injections, dressings and monitoring of vital signs.

We are looking for:

- a registered nurse with a current NMC registration
- A genuine passion for providing care in the community setting and supporting patients to live independently.
- Strong clinical skills, particularly in areas such as wound care, medication administration and palliative care.
- Excellent communication skills and the ability to build relationships with patients, families and team members.
- Ability to work independently and manage your own caseload.
- Full driving licence and own car.
