As a Band 5 Community Staff Nurse, you will manage a defined caseload, using your clinical knowledge and skills to provide safe, effective, and patient-centred care. You will undertake holistic assessments of routine and complex patients, planning and delivering care in partnership with patients and their families. Working largely independently, you will carry out daily visits, document care accurately, and escalate concerns appropriately. You’ll participate in team handovers, liaise with GPs and multidisciplinary colleagues, and support tasks such as equipment ordering. A key part of the role is promoting independence, offering health education, and supporting patients to self-manage. You will work collaboratively across health and social care to ensure patient safety and wellbeing. You’ll also support the development of students and junior staff, acting as a role model and supervisor. Opportunities exist to take on additional responsibilities, champion roles, and deputise for senior staff by coordinating visits and leading handovers.
