Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

### Main duties of the job

Join our Community Nursing Team delivering high-quality care to housebound patients in their own homes. As a Band 5 Community Staff Nurse, you will manage a defined caseload, using your clinical knowledge and skills to provide safe, effective, and patient-centred care.

You will undertake holistic assessments of routine and complex patients, planning and delivering care in partnership with patients and their families. Working largely independently, you will carry out daily visits, document care accurately, and escalate concerns appropriately. You’ll participate in team handovers, liaise with GPs and multidisciplinary colleagues, and support tasks such as equipment ordering.

A key part of the role is promoting independence, offering health education, and supporting patients to self-manage. You will work collaboratively across health and social care to ensure patient safety and wellbeing.

You’ll also support the development of students and junior staff, acting as a role model and supervisor. Opportunities exist to take on additional responsibilities, champion roles, and deputise for senior staff by coordinating visits and leading handovers.

## Person Specification

### Working environment

**Essential**

- A full, valid UK driving licence or a full driving licence issued by an EU country (not exchanged from a non-EU country) and the ability to travel independently are essential for this role.

### Knowledge and Skills

**Essential**

- You will need to have a customer focussed  approach
- Provide patients, clients and their families  with the best possible experience
- You will need to be able to be able to use IT  systems and databases.
- You will understand where to signpost clients and  families to alternative services to support  behaviour change and improve health
- You will have knowledge of up-to-date clinical practice  by using up- to- date skills and adhering to evidence based procedures and clinical effectiveness  frameworks. You will fully understand the role and remit  of a health care worker

### Qualifications, training and experience

**Essential**

- You will be a registered health care professional with the Nursing and Midwifery Council.
- You will have a Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing.
- You’ll need to have recent experience of healthcare gained through education, work experience or previous employment. This role is ideal for a newly qualified graduate nurse.
