An exciting opportunity has arisen for a Band 5 registered professional to work within the West Cumbria CTT.

The Band 5 Community Practitioner is responsible for delivering high-quality, person-centred care within CTT, supporting individuals living with a range of mental health needs in a community setting. The role involves working collaboratively within a multi-disciplinary team to promote recovery, independence, and wellbeing, while reducing hospital admissions and supporting early discharge.

The post holder will manage a defined caseload, providing holistic assessment, planning, implementation, and evaluation of care in line with evidence-based practice and organisational policies. Care is delivered in service users’ homes, clinics, and other community settings, ensuring accessibility and continuity of care.

### Main duties of the job

- Undertake comprehensive mental health and risk assessments, contributing to care planning and review processes
- Deliver therapeutic interventions in line with individual care plans, including psychosocial and recovery-focused approaches
- Monitor depot injections in clinic and community settings, ensuring safe practice and adherence to protocols
- Monitor mental state, medication concordance, and physical health, escalating concerns appropriately
- Develop and maintain therapeutic relationships to support engagement and promote recovery
- Work collaboratively with service users, carers, families, and partner agencies
- Participate in safeguarding processes and risk management, including crisis response where required

### Knowledge and Experience

**Essential**

- Knowledge of relevant contemporary professional theories of mental health care and evidence based interventions.
- Understanding of the statutory issues which impact on service users.

### Education and Qualification

**Essential**

- First Level Registration RMN/RNMH/RNLD Qualification
- Experience of working with service users and carers in successful delivery of treatment plans
- Complete preceptorship programme for newly qualified nurses
- Multi-disciplinary team work.
- Proven ability to develop therapeutic partnership with service users and carers.
- Proven ability to be able to communicate interventions to service users.

**Desirable**

- Further relevant post qualifying training
- Previous experience in a community setting.
- Experience of involving service users/ carers in evaluation/planning and monitoring of services
