The post is a 37.5 hour-per-week substantive post covering the Fareham and Gosport Area, with the understanding that support will be provided to Petersfield CAU from time to time. You will be working within the outpatient Community Assessment Unit clinic setting at Gosport War Memorial Hospital and in the community setting where you will be responding to the urgent care needs of service users in their own homes. This is an excellent opportunity if you enjoy having varied work experience and want to broaden your knowledge and skills in both a clinic and community setting. This role is critical to preventing hospital admissions as part of the Ageing Well agenda and monitoring patients in their own home as part of the UCR virtual ward. As the role will be delivered in patients' homes and clinic-based settings, you will be working closely with all members of the multidisciplinary team. Car owner with a full driving license and business insurance is essential. ### Main duties of the job Upholding all standards as identified by the NMC including the Code of Professional conduct, Documentation and Medicines Management or equivalent according to job title. The post holder will work as an active member of the Community Frailty Team for the Fareham and Gosport Community Assessment Unit and provide a high quality, efficient and effective service to patients. To assist in the delivery of effective and timely assessments for patients who are within the speciality of frailty. To work alongside Consultant Geriatricians, GPs with Specialist Interest, Nurses, Physiotherapists and Occupational Therapists to provide a high standard of individualised nursing care in accordance with agreed policies and utilising an evidence-based approach. To be responsible for the assessment of patients' needs and the development, implementation, and evaluation of planned treatment.
