### Job summary

Hampshire & Isle of Wight Healthcare NHS Foundation Trust has an opportunity for a Community Nurse to join the Community Assessment Unit (CAU) at Gosport War Memorial Hospital and the Urgent Care Response (UCR) team in the community. The post is a 37.5 hour-per-week substantive post covering the Fareham and Gosport Area, with the understanding that support will be provided to Petersfield CAU from time to time.

You will be working within the outpatient Community Assessment Unit clinic setting at Gosport War Memorial Hospital and in the community setting where you will be responding to the urgent care needs of service users in their own homes.

This is an excellent opportunity if you enjoy having varied work experience and want to broaden your knowledge and skills in both a clinic and community setting. This role is critical to preventing hospital admissions as part of the Ageing Well agenda and monitoring patients in their own home as part of the UCR virtual ward.

As the role will be delivered in patients' homes and clinic-based settings, you will be working closely with all members of the multidisciplinary team.

### Main duties of the job

Upholding all standards as identified by the NMC including the Code of Professional conduct, Documentation and Medicines Management or equivalent according to job title. The post holder will work as an active member of the Community Frailty Team for the Fareham and Gosport Community Assessment Unit and provide a high quality, efficient and effective service to patients.

To assist in the delivery of effective and timely assessments for patients who are within the speciality of frailty. To work alongside Consultant Geriatricians, GPs with Specialist Interest, Nurses, Physiotherapists and Occupational Therapists to provide a high standard of individualised nursing care in accordance with agreed policies and utilising an evidence-based approach. To be responsible for the assessment of patients' needs and the development, implementation, and evaluation of planned treatment.

The post holder will be a Registered clinician and will continue to develop their role through the acquisition of further skills, knowledge, and competencies to be determined by the needs of the patients, the service, and the team.

## Person Specification

### Qualifications

**Essential**

- BSc Nursing or RN currently registered with NMC.
- Practice assessor/supervisor.
- Post registration, specialist training relevant to speciality.
- Current (UK/EU) Driving Licence and access to own vehicle for work purposes is required.

**Desirable**

- History Taking and Physical Assessment Course or a willingness to work towards this in the future.
