We have an exciting opportunity for a 8 month fixed term, full time Band 6 Community Mental Health practitioner to join the team at Moss House Community Mental Health Team.

This post is not suitable for newly qualified staff, however it is suitable for qualified Mental Health Nurses, Occupational Therapists or Social Workers.

We are looking for an individual with genuine enthusiasm for working in a community environment, supporting service users to live their life and manage their mental health in order to maximise their recovery and quality of life.

We welcome someone who is caring and who is motivated to provide high quality care with a recovery focused approach for a client group with complex mental health conditions.

The successful candidate will possess excellent knowledge and understanding of severe and enduring mental illness, good communication skills and will work well in a team. The successful candidate will be supported with access to continues professional development, regular clinical supervision and opportunities for personal and professional growth.

### Main duties of the job

The Community Mental Health Team supports individuals who have severe and enduring mental health difficulties. The teams are a busy and skilled multi-disciplinary group which actively promotes close partnership working within the wider community.

If you feel you possess the values of Mersey Care NHS Trust we would welcome an application from you. Applicants should hold the professional qualification of a Registered Mental Health Nurse, Occupational Therapist or Social Worker and have experience working in this field since qualifying.

The role will include providing high quality evidenced based care, formulating risk management plans, supervising risk assessments, providing expertise and advice, care co-ordination and supervising junior staff. In addition, we require a strong commitment to working with service users with complex needs, in order to assist them plan and take steps towards their recovery as well as promoting signposting to additional services such as Psychology, Life rooms and wider resources available in the community.

You will have excellent interpersonal skills that will enable you to support service users and their families and challenge, motivate, influence and inspire your colleagues at all levels.

### Detailed job description and main responsibilities

The post holder will be based within the Community Mental Health Team and will :

Provide assessments and develop treatment plans for service users on caseload

Act as named professional/care coordinator and  coordinate care between services

Hold a caseload of people with severe and enduring mental health problems Participate in team meetings, MDT meetings and training opportunities

Provide advice and support for service users and carers

Participate in delivery of group interventions

Supervision of junior colleagues in the team

Liaise with colleagues and coordinate care for service users on caseload

Support and monitor the service user whilst commencing medication

Work collaboratively with other members of the MDT

Attend regular supervision and team meetings

Participate in training opportunities

Support people with complex needs and provide advice and guidance as people’s needs change
