About the Role:

We currently have a vacancy for a Community Psychiatric Nurse, Social Worker or Occupational Therapist who is:

### Main duties of the job

Our Community Mental Health Teams (CMHTs) are dedicated to providing planned and effective mental health support and interventions tailored to the individual needs and goals of each service user. Care is delivered in partnership with service users, their loved ones, and other agencies to ensure an holistic and person-centred approach.

Collaborative Care:

- CMHTs work closely with other agencies and organisations to promote the health and wellbeing of the local population.
- Responsibility for care is shared between the CMHT, partner organisations, the service user, and their loved ones.

Multi-Disciplinary Team (MDT) Approach:

Our teams operate in a collaborative, recovery-focused way, guided by trauma-informed care principles. The MDT includes:

- Consultant Psychiatrists
- Clinical Psychologists
- Registered Mental Health Practitioners (Mental Health Nurses, Occupational Therapists)
- Community Support Workers
- Peer Support Workers
- Representatives from social care, housing, drug and alcohol services, and the voluntary, community and social enterprise (VCSE) sector

Keyworker Model:

Each service user has an identified keyworker within the team, who acts as their main point of contact and provides continuity of care for both the service user and their carer.

### Skills

**Essential**

- Risk assessment and risk management (e.g. suicide, self-harm, safeguarding)
- Experience of care planning and care coordination

**Desirable**

- Knowledge of mental health conditions and treatment pathways
- Experience working within MDT environments
- Experience of caseload management

### Experience

**Essential**

- Ability to work autonomously and make clinical decisions
- Ability to manage workload and prioritise effectively

**Desirable**

- Experience supporting junior staff / students
- Experience supervising staff

### Registration

**Essential**

- Professional Registration (NMC/HCPC)
- Post-registration experience in a community mental health setting
- Experience managing a defined caseload of patients with complex needs

- Facilitating groups/ workshops to patients, carers and or professionals.
