### Job overview

Community Mental Health Services in City & Hackney are undergoing a significant period of transformation as we move to an integrated Neighbourhood Community Mental Health Model. The new model brings together professionals from across health and social care into local multidisciplinary teams, making services easier to access, better connected, more responsive, and focused on the individual needs of service users.

The service is organised into four geographically aligned neighbourhood teams (quadrants), each supporting people living within their local area. Whilst we are working towards establishing a neighbourhood base within each local area, services are delivered from two main sites, with Vivienne Cohen House supporting the East of the borough and Donald Winnicott House supporting the West of the borough. These vacancies are across the neighbourhood service and successful candidates will be based at one of these locations.

We are seeking enthusiastic and skilled Registered Mental Health Nurses, Occupational Therapists and Social Workers to join our neighbourhood teams as Band 6 Community Mental Health Practitioners. Whilst practitioners will utilise their professional skills and expertise within the role, these posts are primarily focused on care coordination within a multidisciplinary neighbourhood team.

### Main duties of the job

To be an active member of the multi-disciplinary team providing assessment, care and support to people with Mental Health problems within the quadrant mental health teams in City and Hackney.

To conduct comprehensive and high quality assessments – including risk assessments and Dialog + plans.

Manage a caseload, acting as a case manager when required, setting up, implementing and reviewing individualised appropriate care and support plans.

To promote the needs of service users with mental health needs and to offer and provide training and education whenever possible.

To be part of a Duty Rota responding to emergencies and processing referrals.

To assist the Senior Practitioners and the Operational Lead in the  management of the service and deputise in their absence.

## Person Specification

### Knowledge

**Essential**

- Knowledge of mental health legislation, CPA, safeguarding, equality and human rights, clinical governance and professional codes of conduct
- Up to date knowledge of the issues that impact up individuals and carers affected by mental health and or cognitive impairment.
- Knowledge and understanding of approaches to risk assessment and management

### Previous Experience

**Essential**

- Significant post registration mental health experience
- Experience of working effectively as part of a multi-Professional or multi agency team
- Experience of conducting holistic psycho-social assessments and providing comprehensive care and support plans based on individual nee

**Desirable**

- Experience of working in an ethnically - diverse inner city area
- Experience in working in the community.
- Experience of working within an integrated Health and Social Care Team.

### Skills and Abilities

**Essential**

- Ability to communicate effectively both verbally and in writing.
- Ability to prioritise workload.
- Ability to work autonomously and independently.
- Ability to engage with people with serious mental health problems in a holistic manner

### Education/Training/Qualifications

**Essential**

- Appropriate professional qualification (e.g. RMN, AHP, Social Worker)
- Evidence of post registration CPD

**Desirable**

- Higher Education qualification in Mental Health Nursing
- AMHP or willing to undertake AMHP training.
- ENB 998/ Mentorship Course or equivalent (CMHN’s)
