Our team provides a service 8-8pm 365 days a year to older adults experiencing a crisis in the context of functional mental health or dementia. We work intensively with service users in a community setting to prevent admission to a mental health ward, support discharge from a mental health ward and provide comprehensive assessment and treatment plans in the person’s own home. You would be expected to demonstrate the ability to guide, support and develop junior staff with their practice, take a lead in and contribute to service development and effectively co-ordinate and manage the day-to-day workload of the team. In addition, the role involves deputising for the Team Managers in their absence You should be able to demonstrate your passion towards working with this client group and have the ability to assess, co-ordinate treatment and support both clients and carers. You should possess excellent communication skills and have the ability to work under your own initiative and as part of a wider Multidisciplinary team. ### Main duties of the job You will be based in either the dementia or functional sub team however you will be expected to work across both sub-teams at times. As the service is city wide and the need to respond to the needs of clients in a timely manner then it is essential that you hold a full driving licence and have access to a car. The Older Adults Home Treatment Team provides a service 365 days a year to older adults experiencing a crisis in the context of functional mental health or dementia. The team work intensively with service users in a community setting to prevent admission to a mental health ward, support discharge from a mental health ward and provide comprehensive assessment and treatment plans in the person’s own home. The post holder will promote clinical excellence and provide clinical advice and health promotion with nursing staff and other professionals across the Trust and Social Care services. The post holder will work within a multi-disciplinary Older Adults Home Treatment Team (OA HTT) and core responsibilities will include: - Acting as a Lead Professional and Shift Co-ordinator - Working within Care Programme Approach - Comprehensive assessment of new referrals - Carrying a varied caseload defined by need - Delivering supervision to more junior members of staff and students - Lead on and contribute to service development and improvement The post holder will be required to work in accordance with the OA HTT extended working hours. ### Detailed job description and main responsibilities General Requirements - Maintain NMC registration and adhere to professional guidelines and code of conduct - Work within the framework of relevant policy and legislation both local and national Principal Duties Clinical - Triage and determine urgency of new referrals to the service - Carry out comprehensive assessments of an individual’s health and social care needs in a variety of settings and agree the most appropriate course of action - To assess, evaluate and manage clinical risk - To formulate a plan of care based on identified need - Deliver evidence based interventions as appropriate - Hold and manage own caseload consisting of people with more complex needs - To identify, monitor and review physical health and contribute to the management of this - Review effectiveness of care plan at regular intervals - If a non-medical prescribing qualification is held undertake prescribing as appropriate to the needs of the service and in line with the service’s protocols - To work collaboratively with service users, carers and partner agencies - To maintain computer held care records in an accurate and timely fashion - Undertake all work in a culturally sensitive manner - Act in accordance with the Care Programme Approach, Acute and Schedule Care Pathways as appropriate. - Develop effective working relationships with partner agencies Supervision - Regularly review own caseload with supervisor to promote efficacy and contribute to the overall management of the team - Deliver supervision and facilitate annual PDR with more junior members of staff - Participate in own PDR process, identifying training and development needs, and pursuing relevant actions to maintain and improve level of competence and effective contribution Education and Training - Act as mentor to learners, keep up to date with relevant course and curriculum developments, and maintain ‘sign off’ status where applicable - Participate in training relevant to role - Promote insight and understanding of mental health issues to service users, carers and other agencies - Contribute and lead in audit, research and evaluation as appropriate Leadership and Service Development - Take a lead in and contribute to service development and improvement - Deputise for the Team Manager in their absence - Effectively co-ordinate and manage the day to day workload of the team ### Applicant requirements You must have appropriate UK professional registration.
