This is part of the move to integrate secondary mental health care and work closer with primary care and the voluntary sector within the Heeley Plus Primary Care Network. Please note this is a qualified Nursing Vacancy. ### Main duties of the job You will need to possess excellent communication skills and be able to work independently and as part of the wider team in order to assess, plan and evaluate care. You should have a positive approach to meeting the needs of our service users and be motivated, empowered and enthusiastic to both learn and share. You will also likely to be expected to work as part of the duty system which operates within the general working hours of 09:00 to 17:00 Monday to Friday on a rotational basis. ### Detailed job description and main responsibilities The post holder will work within a multi-disciplinary service and core responsibilities will include: - Acting as a Named Worker for service users - Comprehensive assessment of new referrals - Carrying a varied caseload defined by need - Contributing to a duty system on a rota basis - Contribute to Multidisciplinary service development - Conduct male-to-male contacts Clinical - Carry out comprehensive assessments of an individual’s health and social care needs in a variety of settings and agree the most appropriate course of action.
