Working within an Integrated Neighbourhood Team, you will provide advanced clinical assessment, case management, and leadership to support people to remain independent at home and reduce avoidable hospital admissions. You will work collaboratively with GPs, community services, social care, mental health teams, and other partners to coordinate holistic care and improve outcomes for patients and carers. You will need to have the nursing skills, experience, and expertise to assess, diagnose, treat, and evaluate health and needs. All community nurses use EMIS as the clinical record system and are provided with a work Ipad and Iphone. We offer Agenda for Change Terms and Conditions and NHS Pension. As a Community Matron you will be required to work within the Community Coordination Centre on a rotational basis to cover late shifts and weekends, to clinically review and prioritise new, complex, and urgent referrals. Working as part of the Integrated Neighbourhood Team, the post holder will coordinate care across organisational boundaries, deliver comprehensive assessments, and develop personalised care plans that improve patient outcomes and experience. ### Applicant requirements You must have appropriate UK professional registration.
