The position emphasises admission avoidance through rapid response assessments and hospital-at-home interventions, providing patients with alternatives to hospital admission. The service runs from 8am - 8pm which will be required of you to cover. ### Key Responsibilities: - Collaborate with the UCR, Integrated Front Door, and Home-Based Intermediate Care multidisciplinary teams (MDTs) to deliver effective interventions. - Oversee patient care, ensuring a holistic approach to meeting the needs of patients within their homes. - Support from a broad team, including Consultants, GPs, Advanced Clinical Practitioners (ACPs), and the Integrated Care Service (iCares), which spans across Sandwell Community. - Contribute to rehabilitation, case management, and admission avoidance strategies. This role presents an exciting chance to utilise your leadership and clinical expertise in shaping the future of our evolving community services.
