The Gloucestershire Health and Care NHS Foundation Trust is seeking to recruit an enthusiastic, motivated and skilled Community Learning Disability Nurse for the Learning Disability Team.

This post is for individuals working at Band 6 level, however we would also be keen to consider applications from experienced Band 5s looking to develop their skills.

You will benefit from working closely with highly experienced and approachable clinicians in both teams. In addition to The Trust’s excellent mentorship program we will support you through appraisals, mentoring, clinical and peer supervision.

### Main duties of the job

The Community Learning Disability Nurse is a key member of the multi-disciplinary care team and will need experience in community care as the successful candidate will be required to:

- Work independently to ensure timely delivery of a safe and effective service for people who have a range of complex needs in addition to their learning disability, eg mental health, epilepsy, dementia.
- To manage a complex and high-risk caseload over a large geographical area. Linking with other professionals and agencies to achieve positive outcomes for service users.
- To work flexibly based on client need and using reasonable adjustments to provide support to clients, family and carers.
- Ensuring systems and processes are followed in accordance with Trust objectives.

The post holder will be a senior member of the team working with people across the service area who have a Learning Disability and associated complex health needs. The purpose of this role is to:

- Provide assessment and implementation of evidence-based interventions for people with learning disabilities and associated needs.
- Supervise, mentor and teach qualified and unqualified staff including students
- Coordinate care for a specified group of service users

- Support Clinical Governance initiatives at team and service level.

## Person Specification

### Experience

**Essential**

- Demonstrable post qualification experience in learning disabilities or related area, some of which should have been in a community setting
- Extensive experience of first line assessments including the assessment of risk
- Working knowledge of Microsoft Office packages, e.g. Word, Excel, PowerPoint and Outlook
- Experience of collaboration working with other agencies including the voluntary sector

**Desirable**

- Previous experience of autonomous working

### Qualifications

**Essential**

- Registered Nurse Learning Disabilities (RNLD)
- Evidence of continued professional development Essential

**Desirable**

- Formal teaching and assessing course (FLAP / FLIP)
- Registration as a non-medical prescriber with the NMC having undertaken the necessary training at Level 3
- Evidence of formal post registration learning

### Personal Skills and Abilities

**Essential**

- Ability to undertake all statutory and mandatory training including Positive Behavioural Management (PBM)/Breakaway (subject to Occupational Health clearance)
- Approachable and flexible
- Be able to work under pressure and to problem solve in often chaotic and complex situations

**Desirable**

- Ability to engage positively with service users. Ability to work effectively as a team member
- Ability to assimilate new concepts and approaches to care

### Professional/Managerial Knowledge

**Essential**

- Demonstrable skills in risk assessment and management
- Knowledge of evidence-based intervention with service users with Learning Disabilities
- Experience of supervising junior staff and student nurses

**Desirable**

- Ability to identify and positively influence social factors affecting mental distress
- Up to date clinical knowledge including dual diagnosis and health and social care needs
