This is an exciting developmental opportunity under Annex 20 for a band 5 nurse to work towards band 6 competencies.

Experienced band 5 or current band 6 nurses are welcome to apply for either post with prior relevant experience.

Join our friendly Community Learning Disability Team in North Tyneside. Our team is a large well established and supportive multi disciplinary team offering holistic health care to children and adults who have a learning disability living in North Tyneside.

The Ideal candidate will have an understanding of National and Local Policies relating to the management of people with learning disabilities.

Learning Disability student nurses who are due to qualify this year are welcome to apply for this post.

### Main duties of the job

One post will be based in our PBS team carrying out behavioural assessments and interventions for children and adults who present with behaviours that challenge.

The second post will be based in our childrens team, the post-holder will carry out ADHD diagnostic assessments for children who have a learning disability.

Training will be available to successful applicants.

The post holder will be welcomed as a member as a member of the friendly, community learning disabilities team in Northumbria Health Care Trust offering regular team meetings, robust support and supervision, and a commitment to continued professional development.

As an integral part of North Tyneside Community Learning Disability Team, the post holder will be solely responsible for a personal caseload including assessment, planning, implementing and evaluation of care. They will be passionate about working in collaboration with service users, their family, carers and professional colleagues in order to enhance health and wellbeing, to prevent or reduce behaviour that challenges.

### Detailed job description and main responsibilities

- To provide a high quality community nursing service to adults and children who have a learning disability within the North Tyneside area.
- To work across Primary and secondary healthcare services to ensure equality of access for those with learning disabilities.
- To work with other agencies and services (statutory and non statutory) to ensure equality of access for those with a learning disability and facilitate effective communication between clients and other agencies.
- To provide community nursing support to adults and children who have behavioural/mental health problems. Support the design and implement Positive Behaviour Support Plans to provide intensive, proactive support and crisis intervention to patients, carers and care providers to prevent/ reduce admission to in-patient units and to maintain community placements.
- To complete Attention Deficit Hyperactivity Disorder diagnostic assessments for children.

### Qualifications

**Essential**

- Registered Learning Disability Nurse
- Mentorship Qualification or relevant equivalent qualification (essential requirement for post, expectation this will be completed within an 18-month period)
- R.N.M.H/R.N.L.D. Qualification

**Desirable**

- Registered Mentor

### Other requirements

**Essential**

- It is an essential requirement of the role that the post holder has a valid driving licence and is either a car owner and able to use the car for work purposes, or has a Trust personal lease vehicle which may be used for the role. However, the Trust would consider making reasonable adjustments to the role, if necessary, to enable a disabled person to undertake the role

### Experience and Knowledge

**Essential**

- Evidence of ongoing professional development
- Experience working with people who present with challenging behaviours and experience of working within a PBS framework.
- Experience of multi-agency working.
- Experience working with children and adults
- Experience working in the community setting
- Evidence of qualification to undertake the supervision, teaching and assessing of students (essential requirement for post, expectation this will be completed within an 18 month period).

**Desirable**

- Knowledge of or experience in coaching and mentoring practices and tools.
- Knowledge of or experience in Quality improvement tools, techniques and methods.
