The nursing team is a strong, passionate and dedicated team of RNLD’s who endeavour to improve the health outcomes for people with learning disabilities across Ealing. We are looking for a full time band 6 Learning Disabilities Nurse to join us.

The successful candidate will be responsible for implementing the nursing model to assess, plan implement and evaluate care, ensuring people’s physical health needs are met; to support mainstream health services to meet the needs of people with learning disabilities and to support hospital admission and discharges.

The successful candidate will be the clinical and operational supervisor for a junior member of the team, and support student nurses on placements within the team.

You will be provided with many exciting opportunities to further enhance your learning and development through nursing away days, annual trust nursing conferences, attending trust quality awards and training and development opportunities.

Flexible working opportunities available, including agile working. Queries are welcomed via discussion with the appointing manager.

### Main duties of the job

Under taking initial nursing assessments

Care planning and implementation

Supporting mental capacity assessment and best interest meetings

Supporting family members, carers and providers

Health promotion

Health facilitation

Implementation and development of care pathways

Liaison with external stakeholders

Support service users to manage their physical health needs:

Supporting GP’s to increase annual health check uptake and quality

Facilitating blood tests via desensitisation programmes

Epilepsy care plans

Supporting the dementia pathway

Symptom management and end of life care planning

Supporting weight management

Promoting positive sexual health

Promoting healthy lifestyle choices

Partnership working with the MDT:

Attending weekly referral meetings

Attending weekly case discussion meetings

Raising safeguarding concerns

Reporting incidents

Management:

Clinical and operational management of a band 5 nurse

Supporting student nurses in the role of practice supervisor and assessor – additional training available

Attending regular supervision

Adhering the NMC requirements inc. revalidation

- To work independently with minimal supervision as well as being able to work well as an integral team member

### Knowledge and Experience

**Essential**

- A minimum of 1-year post graduation experience in working as a learning disabilities nurse
- Good understanding of the needs of people with learning disabilities.
- Experience of managing the physical health needs of people with learning disabilities.
- Basic understanding of mental health needs of people with learning disabilities
- Knowledge and understanding of health care systems and structures.

**Desirable**

- Experience of working in a community setting
- LeDeR training
- Experience of safeguarding
- Experience of facilitating training
- Experience of working with the Mental Capacity Act
- Experience of lone working
- Supervision of junior members of staff
- Previously supported student nurses on placements within an LD nursing setting
