We have a fixed term vacancy opportunity, for someone to join our team to continue strengthening community support for adults with Learning Disabilities, in the west community team in Cornwall.

This   Community Nurse post joins an established nursing and MDT team in west Cornwall. You will undertake nursing duties aligned to agreed Care Processes and Pathways, working closely with individuals, families and carers to deliver person‑centred assessment, intervention, monitoring and review. You’ll provide evidence‑based practice within an innovative multidisciplinary team supported by supervision and appraisal. The team and wider trust offer CPD, research, preceptorship programme and audit opportunities, with excellent service‑user engagement and joint working across Acute and Primary Care liaison, Social Care, Mental Health, Forensic and Children’s services.

Clinical work includes multidisciplinary assessment of complex cases, formulation and delivery of therapeutic interventions, planning and supervising work for HCAs, and providing patient‑centred training packages to care providers.

expected health outcomes.

**Essential**

- Demonstrating a positive and valuing approach to individuals and their families and carers
- Able to work autonomously, jointly with colleagues and as part of a team
- Communicate effectively with colleagues, individuals with a learning disability, their family and carers

### Knowledge and experience

**Essential**

- Relevant clinical experience in a community or health care setting
- Understand the impact of health inequalities
- Understanding of relevant national guidance, policy drivers and evidence- based practice

### Education / Qualifications

**Essential**

- Professional Registration with Nursing and Midwifery Council – RNLD/RMN/RN
- Completion of, or willingness to complete, relevant preceptee programme if appropriate
