We are looking for Community Learning Disability Nurses.   Join an established nursing and MDT team within the west of Cornwall.  Be responsible for undertaking nursing duties in line with agreed Care Processes and Pathways. Work actively with the individual and their families and/or carers providing person-centred, skilled and effective  assessment, intervention, monitoring and review.  Be responsible for delivering evidence-based practice, within an innovative, multi-disciplinary team with a robust supervision and appraisal culture. There are  CPD, research and audit opportunities within the team and wider trust. There are good levels of service user engagement and joint working with other teams including Acute and Primary Care liaison, Social Care, Mental Health, Forensic and Children’s, etc  Clinical work is multi-disciplinary involving assessment of complex cases, formulation and provision of therapeutic interventions,  as part of the multi-disciplinary care planning and delivery. Planning, directing and supervising associated work for HCAs and delivery of patient-centred training packages to care providers.

### Knowledge and Abililties

**Essential**

- Relevant clinical experience in a community or health care setting
- Understand the impact of health inequalities

**Desirable**

- Work as part of a multi disciplinary team for individuals with complex needs

### Education / Qualifications and Relevant Experience

**Essential**

- Professional Registration with Nursing and Midwifery Council – RNLD/RMN/RN

**Desirable**

- To act as a mentor for learners. Having the mentoring qualification or being willing to work towards this qualification and attending mentorship updates as required.
