The post holder will deliver a high-quality Community Nurse Service to people with a learning disability who meet the service criteria within their allocated area. They will have responsibility for assessing care needs; the planning, implementation and evaluation of nursing care plans within a multi-disciplinary community team structure.

The post holder will work as an integral member of the learning disability community team in the delivery of a comprehensive package of care. To implement the Improvement Cymru action plan within their working day. To facilitate a smooth transition from Children’s service into Adult Learning Disabilities services.

- Carry out holistic nursing assessments and deliver person-centred interventions
- Build and maintain therapeutic relationships with people with learning disabilities
- Prioritise, manage and organise a caseload in a community setting
- Identify, assess and respond appropriately to clinical risk
- Work both autonomously and effectively within a multidisciplinary team
- Adapt communication to meet diverse and complex communication needs
- Motivated to engage in reflection, supervision and continuous professional development
- Exercise sound nursing judgement
- Assess clients, develop and monitor care plans
- Establish feedback processes from referrers, people using service and providers

**Desirable**

- Some ability to speak, read and/or write Welsh, or an eagerness to learn
- Contribute to audit, quality improvement or service development projects
- Support health promotion activities tailored to people with learning disabilities

### Experience / Knowledge

**Essential**

- Significant experience supporting people with learning disabilities in any care setting (including during training/placements)
- Significant experience of working with individuals with complex health needs and those with challenging behaviour
- Contributing to the assessment, planning, delivery and evaluation of nursing care
- Strong verbal and written communication skills, with the ability to explain complex or sensitive information clearly
- Managing changing priorities in a clinical environment, including responding to urgent or unexpected situations
- Risk management
- Experience/knowledge of the Mental Health Measure Wales
- Working with families, carers and support providers

**Desirable**

- Working within a community learning disability team or community-based service
- Supporting individuals with complex needs, including behaviours of concern, mental health needs, or dual diagnosis

### Qualifications & Skills

**Essential**

- Registered Nurse (Learning Disability) with current Nursing and Midwifery Council registration
- Postgraduate level in specialist area
- Evidence of ongoing professional development relevant to nursing practice
- Understanding of the health needs of people with learning disabilities, including associated health inequalities
- Knowledge of person-centred care and the importance of therapeutic relationships
- Awareness of safeguarding responsibilities for children and adults and how to escalate concerns
- Understanding of risk assessment and clinical risk management within community settings
- Knowledge of relevant legislation, including the Mental Capacity Act, Equality Act, and Human Rights-based approaches
- Understanding of multidisciplinary and multi-agency working in community services
- Knowledge of accurate record-keeping and digital documentation requirements
- Completed PBM training

**Desirable**

- Practice Supervisor and Practice Assessor qualification (or willingness to undertake)
- Additional training relevant to learning disability nursing (e.g., Positive Behavioural Support, sensory needs, physical health skills)
- Training in behaviour support, mental health awareness, or communication approaches for people with complex needs
- Awareness of trauma-informed practice and its relevance to people with learning disabilities
- Awareness of public health initiatives relevant to people with learning disabilities
