### Job summary

We are seeking to appoint an enthusiastic Community Hub Coordinator to support our teams and service users, within Community Health Services for Nursing and Therapy based primarily at St Luke's Hospital in Market Harborough, with weekends and occasional shifts from Springfield Road Health Centre in Leicester (Due to service needs, including travel between dual sites and limited public transport availability in rural locations, applicants must hold a full driving licence and have access to a vehicle. Free parking is available).

Our service operates 08.00 to 18.00, Monday to Sunday, therefore this post requires the candidate to be fully flexible and be able to cover all shifts (including weekends and bank holidays, these shifts attract enhancements).

### Main duties of the job

The role requires strong communication skills and the ability to work independently across community settings, while maintaining flexibility and contributing effectively within a team. You must be able to use your initiative and adapt to changing demands.

Duties include general administrative tasks and use of the electronic patient record system (SystmOne). Responsibilities involve processing referrals, managing tasks, supporting data collection for audits, and ensuring accurate data entry. The post supports Community Nursing and Therapy teams, Team Leads, and clinicians, including ordering and monitoring equipment and supplies. This list is not exhaustive. You should be highly organised, a strong team player, and demonstrate excellent communication skills.

Qualifications & Experience

- GCSE grade C or above (or equivalent) in English and Maths
- NVQ Level 3 in Business Administration (or equivalent)
- Experience in the NHS or a similar complex organisation
- Background in a customer service environment
- Experience handling varied and changing administrative demands

- Administrative/clerical experience
- IT literate, including SystmOne, Word, Excel, PowerPoint, email and internet
- Ability to multitask and work under pressure
- Strong verbal communication skills
- Able to work independently and use initiative
