An opportunity has risen for part time 3 days per week Heart Failure Specialist Nurse to join our Community Cardio Respiratory Team in Mid Notts, supporting patients with a diagnosis of Heart Failure. Ensuring patients' heart failure medication is titrated and optimised by safely prescribing the four pillars of heart failure treatment. You will be expected to support caseload management of patients with Heart Failure, proving care plans as appropriate, working closely with members of the Cardio Respiratory Team and the wider MDT- including our acute colleagues, GPs and other health professionals in order to maximise patients quality of life and encouraging self management. This care takes place in community clinic settings and patients own homes /usual place of residence. The service is a 5 day service operating from 8.00-5.00pm, due to the nature of this role, it is essential that you are a UK car driver / driver licence holder and able to travel independently across the county and have use and access to a suitable vehicle for business purposes. ### Detailed job description and main responsibilities Are you a registered Nurse Passionate about providing care for patients with chronic Heart Failure? An exciting opportunity has arisen for part time (3 days per week) Heart Failure Specialist Nurse to join our excellent Community Cardio Respiratory Team in Mid Notts, helping to support caseload management of patients with a diagnosis of Heart Failure. This role is will cover caseload in Mansfield and Newark within the Mid Notts District. Providing Care and treatment for patients who have been diagnosed with heart failure with a reduced ejection fraction. You will be expected to support caseload management of patients with Heart Failure, proving care plans as appropriate, working closely with members of the Cardio Respiratory Team and the wider MDT- including our acute colleagues, GPs and other health professionals in order to maximise patients quality of life and encouraging self management. This care takes place within community clinic settings and patients own homes /usual place of residence. We are looking for a passionate, dynamic and flexible individual who are able to adapt to the changing demands and workload. The service is a 5 day service operating from 8.00-5.00pm, due to the nature of this role, it is essential that you are able to travel independently across the county and have access to a suitable vehicle for business purposes. Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies. A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010. ### Applicant requirements You must have appropriate UK professional registration.
