We're looking for dedicated individuals to join our thriving Community Nursing Service across Hampshire in a permanent role, delivering outstanding care to the community in a supportive and collaborative environment.

Our Community Nursing Service provides high-quality nursing care to patients in their own homes, operating 8am--8pm, 7 days a week.

We pride ourselves on delivering person-centred care and ensuring our staff feel valued, supported, and encouraged to develop their skills. In this role, you'll work as part of a wider multi-disciplinary team, contributing to care planning, evaluation, and effective communication across services.

A full driving license and access to a vehicle is required for this role. There is a space later in application to put your preferred location.

At present, the Trust is unable to offer Skilled Worker visa sponsorship for this post. Applicants must have the right to work in the UK at the time of application

### Main duties of the job

We have various locations to offer including Bitterne, Southern parishes(includes West End, Eastleigh & Netley), Hythe & Waterside and Portsmouth.

Please note Waterside vacancy is part time of 3 days a week(22.5hours).

The post holder will have responsibility for participating in the delivery of patient care under the supervision of the Community Team.

*Work under supervision following written care plans and care pathways devised by the Registered Nurse.

*Follow prescribed care plans and treatment protocols.

*Follow HPCT Policies and Procedures.

*Has knowledge and skills to carry out prescribed tasks and will report to Registered Nurse any variances.

*Delegated responsibility for Patient Support Teams administrative tasks.

*Provide general administration support to the team.
