We are looking for an enthusiastic Community Healthcare Support Worker for the Centre House Nursing Team, to provide care and support for patients in the community setting as delegated by the registered nurse.

Access to a motor vehicle is essential , with appropriate driving license. Please ensure that your application demonstrates that you meet the essential criteria as listed in the job specification.

This role is 4 day shifts a week.

Please note: There will be a numeracy and literacy test, candidates who score the passmark will be interviewed. Both the test and interview will be conducted face to face.

### Main duties of the job

To work within the District Nursing team to provide care and support for patients in the community setting as delegated by the registered nurse. You will be expected to carry out all relevant forms of nursing care including phlebotomy , without direct supervision, and may be required to demonstrate simple procedures to qualified and/or unqualified staff undertaking training where agreed with the nursing team.

- IT literate with ability to use databases and other software packages
- Strong written and verbal communication skills, with the ability to produce clear, accurate and timely clinical records.
- Basic clinical skills e.g blood pressure, temperature, pulse, urinalysis, blood glucose monitoring

**Desirable**

- Developed clinical skills, e.g medicines administration, insulin injections, venepucture, continence care, wound care

### Experience

**Essential**

- Experience of providing care in a health or social care related setting

**Desirable**

- Working within a community setting
- Experience of working in a healthcare environment

### Qualifications

**Essential**

- Educated to GCSE Standard; A-C including English and Maths
- Practical knowledge acquired through, training, experience or NVQ level 3 or equivalent in a health care social setting
