\<p\>\<strong\>Be the First Point of \<span class="single_desc_int_gb_word"\>Support\</span\> That Makes a Difference\</strong\>\</p\>\<p\>We are looking for motivated and customer-focused \<strong\>Community Health Navigators\</strong\> to join our BSW (Bath & North East Somerset, Swindon and Wiltshire) Community Health Services \<strong\>Single Point of Access (SPA)\</strong\> team.\</p\>\<p\>As a Community Health Navigator, you will play a vital role in helping patients access the right care and \<span class="single_desc_int_gb_word"\>support at the right time\</span\>. Acting as the first point of contact for patients, members of the public, healthcare professionals and colleagues, you will provide a professional, \<span class="single_desc_int_gb_word"\>compassionate and efficient service across a range of community health services\</span\>.\</p\>\<p\>Working within a busy and \<span class="single_desc_int_gb_word"\>supportive team, you will handle enquiries via telephone, email and post, providing information, guidance and signposting to appropriate services\</span\>.
