We are looking for enthusiastic Healthcare Assistants to join our Community Nursing Service in a new role that supports care delivery across Buckinghamshire. Working as part of a supportive team, you will provide direct patient care and assist with long-term condition reviews, phlebotomy, continence care and other delegated clinical tasks. You'll work closely with Community Nurses and other healthcare professionals to help people remain independent at home, avoid unnecessary hospital admissions and achieve the best possible outcomes. This is an excellent opportunity for Healthcare Assistants who are keen to develop their clinical skills and be part of an innovative approach to community healthcare. We offer ongoing training and support within a 7-day service. ### Main duties of the job - To work alone using initiative and a wide range of health and social care skills, within the guidance set and agreed by the professional members of the team. The term ‘professional’ member of the team refers to those clinicians, social workers etc within the team who make the initial assessment and are responsible for the patient’s case management. - To prepare for and provide care and interventions enabling patients in their own home or community based facility.
