Do you want to make a difference to someone’s ability to remain safe and independent in their own homes?

Are you educated to NVQ Level 3 or Equivalent and have experience of working in the community as a health care assistant, reablement support worker or in an occupational therapy or physiotherapy setting?

The service works shifts from 8pm until 6pm, 7 days per week with weekend work on a rota basis. The applicant would be expected to work shifts across. A typical shift pattern for a full time member of staff may include 4 working days over a 7-day period with 3 rest days.

We have an exciting opportunity for motivated, well organised and enthusiastic individuals to join our falls team as a Part time Community Falls Support worker here in Luton. We are a small proactive team that work autonomously using your own initiative, have good IT and communication skills. Full support and training will be provided and the Trust is committed to staff development.

The team works with people who have fallen or are at risk of falling in their own homes and aims to prevent unnecessary hospital admissions and reduce the risk of falls.

### Main duties of the job

- To work within the Falls Team in providing timely intervention service for people who have had a fall or are at risk of falling.
- To complete a comprehensive falls assessment and identify any equipment, care or support required to enable the patient to remain at home safely and avoid unnecessary hospital admission.
- To facilitate/support the delivery of falls strength and balance classes across Luton, therefore there will be a requirement to travel locally between community group venues, clinics and patients own homes.
- To make Falls prevention recommendations and advice and completed onward referrals to relevant stakeholders as appropriate.
- To work closely with GP suregeries, Luton integrated community services, the ambulance service and other health, social care and voluntary organisations in Luton to reduce falls.

### Detailed job description and main responsibilities

- To respond quickly to all referrals for patients who have fallen or at risk of falling, arranging to visit them as soon as possible.
- To visit patients in their own home or place of residence.
- To complete a comprehensive, evidence-based falls assessment and identify any equipment, additional care or support required to enable the person to remain at home safely.
- To support the Luton Falls Team in delivering all aspects of the falls strength and balance classes/ group programme as necessary to include: patient reviews, exercise assessment, education and psychological support.
- To support delivery of all elements of the falls strength and balance group program: namely- face to face groups, virtual sessions both live and pre-recorded, and home exercise sessions.
- To analyse facts and make judgements within own competency level. This will involve the use of skills to evaluate the patient’s condition and adapt the assessment and recommendations as necessary.
- To work with service users and their carers to design packages of care that meet assessed needs, promote independence and choice, respect service users’ dignity and rights and meet cultural needs. To communicate information to the relevant services to support the smooth implementation of care.
- To assess for, provide and demonstrate basic mobility equipment and equipment to aid activities of daily living.
- Based on the falls assessment, to make recommendations to patients and their carers that will reduce the risk of further falls.
- To make referrals to other services where this will reduce the risk of further falls and promote patients’ independence.
- To follow up patients to ensure that equipment has been delivered and care is in place.
- To maintain and update patient records in a timely manner using an electronic system and to work closely with the Falls Coordinator to collate and analyse associated data.
- To undertake related administrative duties – completing notes and onward referrals in timely manner to Social services, safeguarding, Community rehab, Assistive Technology Services, Age concern, Total Wellbeing, Fire Service, Medequip and other specialist services with EEC
- To be responsible for planning and prioritising own workload.
- To work autonomously according to service protocols, procedures and pathways.
- To contribute to the development of efficient operational procedures and local pathways and participate in service development projects and initiatives. To comment on proposals for changes in own area.
- The post holder may be required to organise and run group activities.
- To support team Lead in completing Audits
- To support team in promotion activities within Luton through other service partners

### Skills

**Essential**

- Effective communication skills, written, verbal, report writing skills
- Time management, ability to organise and prioritise own work
- Car owner and UK licence driver

**Desirable**

- Skills in providing education and advice
- IT Skills in using Excel/Word/Powerpoint

### Experience

**Essential**

- Experience of working in the community setting/Home visits
- Experience in facilitating strength and balance groups
- Equipment risk assessment, ordering and installation of assistive devices and mobility aids

**Desirable**

- Experience in SystmOne or other similar electronic patient records
- Working in Falls related cases

### Training and Education

**Essential**

- NVQ level 3 or Degree or equivalent, e.g. BSc,MSc or Diploma entry level qualification into the Physiotherapy profession.

**Desirable**

- Training related to Falls prevention
- Training related to Strength and balance exercises/OTAGO/Taichi/FaME
