The post is based at Honeypot Lane Health Centre and involves working within a well-established multidisciplinary team to deliver high-quality, evidence-based diabetes care across community settings. The successful candidate will provide specialist clinical expertise to support adults living with diabetes, promoting effective self-management and improving patient outcomes. The role requires both autonomous practice and close collaborative working with primary care, community services, and secondary care colleagues.
