### Join Our Community Diabetes Service in Harrow

An exciting opportunity has arisen for an experienced Band 7 Community Diabetes Specialist Nurse to join the Community Diabetes Service in Harrow. The post is based at Honeypot Lane Health Centre and involves working within a well-established multidisciplinary team to deliver high-quality, evidence-based diabetes care across community settings.

The successful candidate will provide specialist clinical expertise to support adults living with diabetes, promoting effective self-management and improving patient outcomes. The role requires both autonomous practice and close collaborative working with primary care, community services, and secondary care colleagues.

In addition, the post holder will contribute to service development initiatives, deliver structured education to patients and healthcare professionals, and support the ongoing improvement and transformation of community diabetes services within Harrow.

### Main duties of the job

- Support the Team Leader in delivering a high-quality specialist Community Diabetes Nursing Service, contributing to service planning and delivery in line with Trust objectives.
- Provide expert clinical assessment, management, and review of patients with diabetes, ensuring safe, evidence-based, and patient-centred care.
- Work collaboratively as part of the multidisciplinary team, liaising with primary, secondary, and community care services to ensure coordinated care pathways.
- Lead and support the development, implementation, and evaluation of specialist nursing interventions and service improvements.
- Use project management and clinical leadership skills to contribute to service development aligned with national guidance, local priorities, and best practice.
- Maintain accurate, timely, and comprehensive clinical documentation and records in line with professional and organisational standards.

### Experience

**Essential**

- Minimum of at least 3 years’ experience in managing complex diabetes cases.
- Experience in managing a clinical case load.
- Experience in the training of students.
- Experience in organising and implementing training/teaching. programmes for healthcare professionals and clients
- Excellent group education and presentation skills.
- Experience of conducting audits and/or research.

### Education/Qualification

**Essential**

- Degree in Nursing or equivalent.
- Current registration with the NMC, Showing a registered General Nurse
- Relevant post registration training in Diabetes eg: Masters or diploma in diabetes management.
- Independent Nurse Prescriber (or willing to undertake)
