### Job summary

Are you a nurse or OT who is passionate about supporting people living with dementia?

Are you able to provide interventions to both patient and carers to maintain wellbeing and prevent crisis?

If so, come and join our innovative Community Dementia Service.

Our Community Dementia Service is provided in collaboration with the Alzheimers Society and will form a key part of support throughout the dementia journey. Sitting within our Older Adult Community Mental Health Team (OACMHT), you will be providing brief interventions for people living with dementia with the aim of reducing distress, maintaining wellbeing and avoiding crisis. You will have access to the wider MDT where patients can be stepped-up to the OACMHT for more complex or on-going support. You will also spend time each week undertaking routine dementia assessments or other OACMHT activities.

Working hours are Monday to Friday 09.00-17.00. Part time and flexible working will be considered.

### Main duties of the job

A typical day will include a mix of direct clinical assessment and interventions for people living with dementia as well as providing clinical advice to the Dementia Advisors and other services e.g. Primary Care. You will offer short term interventions to patients and their carers to reduce distress, maintain wellbeing and avoid crisis. Your interventions will be goal based and look to empower those accessing the service. You will support the development of the service through the collection of outcome measures and other metrics and engage in promotion and education events, as well as your own professional development. You will provide a seamless transition for patients supported by the Dementia Advisors into your service and utilise your expert knowledge to assess need and guide patient care. You will take a collaborative approach to all of your work, and ensure accurate documentation is maintained.

## Person Specification

### Knowledge

**Essential**

- Knowledge of principles of assessment of people with mental health problems
- Knowledge of principles of risk assessment and management
- Knowledge of Mental Health Act 1983 and Mental Capacity Act 2006 and other relevant legislation

### Experience

**Essential**

- Experience of working in a Mental Health setting
- Assessment of people presenting with mental health problems
- Assessment and management of clinical risk
- Working within a multidisciplinary team
- Managing own case load

**Desirable**

- Working in collaboration with VCSE service
- Experience of Care Coordination
- Training/developing staff and / or patients
- Experience of delivering evidence based psychosocial interventions

### Qualifications

**Essential**

- Relevant professional qualification relating to Health or Social Care e.g., in social work, nursing, OT, with current professional registration
- RMN, BSc/DipSW, BSc OT
- Recent evidence of continuing professional development
- I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.

**Desirable**

- Teaching/training qualification
- Psychological therapy qualification

### Skills & Abilities

**Essential**

- Ability to communicate clearly and effectively in English in both verbal and written formats
- Mental health assessment skills
- Care planning
- Delivery of evidence-based interventions in mental health

**Desirable**

- Skilled in the provision of clinical supervision to both peers and junior members of staff

### Additional Criteria

**Essential**

- Able to meet the travel requirements of the post.
