### Job overview

To work as a member of the Continence & Urology Team to provide high quality, evidence-based, holistic nursing care to patients within a clinic, care home or community setting.

To assess, plan, implement and evaluate care needs using a person centred approach.

This is a well established team who provide Specialist Continence Assessment and treatments for Children and Adults across the whole of the West Cheshire Footprint.

### Main duties of the job

- To carry out continence assessments, implement and evaluate treatment programmes to patients in the community and care home settings.
- Professionally responsible and accountable for own caseload.
- Participate in nurse led continence clinics,
- Perform clinical skills relating to the post.
- Education and training to carers.
- Responsible for maintaining notes and ensuring appropriate storage of confidential records and record keeping.
- Work with external agencies to raise profile of continence promotion.
- To communicate effectively with relatives, clients, agencies and the multi-disciplinary team.s
- To act as the clients advocate at all times.
- To encourage good relationships with other statutory and voluntary services involving collaborative working.
- To have good communication within the continence team.
- Responsible for managing and prioritising own caseload.

A small annual fee of £16 per year applies.

You must have appropriate UK professional registration.

This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
