Are you passionate about working with Children and Young people with moderate to severe learning disabilities? The Children’s Community Learning Disability team in South Staffordshire are expanding and have an exciting opportunity for a creative and innovative Specialist Nurse who has experience of working with children who have a learning disability and their families. In the role you will have the opportunity to work alongside an experienced group of Learning Disability Nurses, Psychologists, Occupational Therapist, SLT and Psychiatrist. In MPFT Children and Families Mental Health Function, we work with young people who have a wide range of needs using an inclusive, person-centred approach. Our model places our young people’s needs at the heart of what we offer to ensure that they are getting the best care possible. The Children’s Learning Disability team offers: support and intervention for young people with emotional and physical healthcare needs who may be at risk of placement breakdown or hospital admission. - You will be responsible for the management of a complex caseload, which includes, specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans for CYP with learning disabilities. ### Main duties of the job - Be responsible for the assessment of mental health and/or medical risk factors relating to children and young people, including co-morbidity (e.g. suicidality, self-harming behaviours and medical risk associated with mental health issues) - To undertake physical health monitoring as appropriate i.e height, weight, blood pressure etc and be responsible for identifying appropriate action to be taken - Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance. - Complete and contribute robust and dynamic risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve and offer optimal quality of life and health outcomes - Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions. - Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process. Evidence of extensive post registration experience with working with children & young people with learning disabilities - Experience of working with people who have difficulty in engaging with services and have complex mental health needs - Experience of working in community setting and making decisions autonomously - Experience of managing health needs related to medication optimisation - Experience of assessing, planning, co-ordinating and managing people with mental health problems in a community setting - Experience of integrating care delivery across multi-disciplinary /multi agency teams - Experience of leading/contributing to CPA - Experience of dealing with mental health related issues/referrals/need for advice by telephone including at times of high demand and in relation to highly stressful/emotive situations. **Desirable** - Experience of presenting at local/national level - Experience and ability to carry out clinical audit - Experience of undertaking physical health monitoring i.e Height , weight, Blood Pressure ### PERSONAL ATTRIBUTES **Essential** - Registered learning Disabilities Nurse (RNLD) or Mental Health Nurse (RMN) - BSc degree in nursing or equivalent level - Post-registration experience, evidence of formal courses and training at diploma/degree level. - Conforms to NMC requirements **Desirable** - Leadership qualification - Qualification in providing supervision - Qualification in coaching - Psychological intervention Qualification including EMDR, CBT, CBTp, Family Interventions, CAT, DBT - Registered nurse mentor/supervisor - Additional medication management/optimisation training ### SKILLS, KNOWLEDGE & ABILITIES **Essential** - Knowledge of the NICE Guidelines in relation to Learning disabilities and CYP - Understanding of the principles of the Assessment, formulation and care planning for CYP with learning disabilities - Demonstrate understanding of Equality and Anti-discriminatory practice - Knowledge of other relevant local and national documents/drivers that affects practice - Skills of assessing, planning and evaluating care, for CYP with learning disabilities or Mental Health needs - Skilled in the assessment of clinical risk and developing and communicating appropriate risk management plan - Knowledge and experience of holistic assessment of someone’s mental ill health, strengths and needs and appropriate nursing and multi-disciplinary and agency interventions. - Ability to complete and interpret Clinical Outcome measures such as CORC - Ability to integrate the assessment, care planning and the review process for CYP/families and carers in a sensitive manner - A range of skills and knowledge to deliver evidence based interventions - Skilled in engaging those who ordinarily may find accessing services challenging - Range of Interpersonal skills, (communication, listening, working collaboratively) maintaining good working relationships with service users/carers, other service providers and agencies - Ability to manage staff/support workers and to incorporate management /leadership skills into practice including: promoting positivity, supporting innovation, management of performance, delegation and caseload and clinical supervision. -
