### Job summary

The Community Charge Nurse will play a key leadership role in managing a team of community nurses to deliver high quality, compassionate care to patients in their own homes. ### Main duties of the job

The post holder will work autonomously within their professional scope, taking responsibility for all aspect of community health care delivery and the safe day to day running of the service. They will line manage a team of staff, support the management of an active caseload, and oversee staff rostering. They will also participate in the triage rota, carrying delegated authority to admit, refer and discharge patients. The Community Charge Nurse will be responsible for incident management, including reviewing incident forms and ensuring appropriate follow up and learningThey will actively contribute to the Primary Care Network (PCN), involve the wider MDT where appropriate, and delegate tasks to ensure safe and effective care. Th role includes providing clinical supervision to new and existing staff, working within legal and ethical frameworks, and adhering to organisational policies. The post holder will also support quality improvement by identifying opportunities for service development within safe and appropriate boundaries

## Person Specification

### Experience

**Essential**

- Experience of working with long term conditions and frailty
- Evidence of innovative practice
- experience, underpinned by knowledge of working with and understanding the complex needs of patients in a primary care/community setting
- experience of caseload management including responsibility for complex care packages for vulnerable people

**Desirable**

- Experience of initiating or participating in clinical audit/research relating to clinical practice

### Qualifications

**Essential**

- Registered Nurse
- Post registration qualification or university degree

**Desirable**

- Management/ leadership qualifications/ development programme
- Teaching and assessing qualification.

- Excellent communication and interpersonal skills
- Broad range of enhanced clinical skills
- Ability to demonstrate leadership skills
